摘 要:目的研究患者血清C反应蛋白(CRP)值的高低与脑梗死诊治之间的关系,探索CRP在脑梗死治疗中的临床价值及意义。方法分析某院内科初次明确诊断为脑梗死的112例患者,测定其发病d3血清CRP水平,根据CRP水平,将患者分为低CRP组和高CRP组,入院后对所有患者进行神经功能缺失评分,并计算发病后3月的残疾率、死亡率。测量(或计算)两组患者颈动脉粥样硬化各相关指标。结果低CRP组患者占42.86%,高CRP组患者占57.14%。低CRP组患者的神经功能缺失评分、残疾率及死亡率分别为(10.48±3.96)分、37.50%、8.33%,明显低于高CRP组的(19.14±9.01)分、73.44%、23.44%;低CRP组患者颈总动脉内膜中层厚度(IMT[Microsoft1])、颈内动脉IMT、颈动脉斑块发生率分别为(1.17±0.14)mm、(1.07±0.10)mm、54.17%,明显低于高CRP组患者的(1.29±0.10)mm、(1.21±0.09)mm、85.94%;低CRP组共发现斑块101处,高CRP组共发现斑块219处,低CRP组颈动脉粥样硬化斑块患者稳定性斑块构成比57.43%,明显高于高CRP组的38.81%,差异有统计学意义(P﹤0.05)。结论应加强脑梗死患者CRP值的监测工作,以有效评价脑梗死患者病情,积极进行相应的治疗。OBJECTIVE To study the value of serum C-reactive protein (CRP) level in patients with cerebral infarction and the relationship between them. To explore the clinical significance of CRP in the treatment of cerebral infarction. METHODS The clinical data of 112 patients with cerebral infarction were retrospectively analyzed in our hospital. The serum level of CRP on the third day was determined. The patients were divided into low CRP group and high CRP group based on the result. The neu- rological deficit score, the disability rate and the mortality rate after three months were calculated and compared. RESULTS The patients in low CRP group accounted for 42.86%, while 57.14% in high CRP group. Compared with that of high CRP group (19.14± 9.01), the neurological function deficit score in low CRP group was (10.48±3.96), which showed significant differ- ence (P〈 0.05); Compared with that in the high CRP group (73.44% and 23.44%), the rate of disability and mortality in low CRP group were 37.50% and 8.33%, which showed significant difference (P〈 0.05); The carotid artery IMT, internal carotid artery IMT and carotid artery plaque in the lower CRP group were (1.17 ± 0.14) ram, (1.07±0.10) mm and 54.17%, re- spectively, while they were (1.29 ± 0.10) mm, (1.21± 0.09) mm and 85.94% in the high CRP group, which showed sig- nificant difference between two groups (P 〈 0.05 ). 101 plaques were detected in the low CRP group, while 219 in the high CRP group; The unstable plaques accounted for 57.43% in low CRP group, while the value in high CRP group was 38.81%, which showed significant difference between both groups (P 〈 0.05 ). CONCLUSION The CRP monitoring in patients with cerebral in- farction should be emphasized to effectively evaluate their symptom, and push the therapy correspondingly.
