重症肌无力住院患者发生肺部感染的影响因素分析

摘　　要：目的探讨重症肌无力(MG)住院患者发生肺部感染的影响因素。方法收集某院2006年1月～2012年4月收治的193例重症肌无力患者的临床资料,分析住院患者发生肺部感染的影响因素。结果 193例重症肌无力患者住院期间共有27例(17.53%)发生肺部感染。单因素回归分析结果显示,年龄、发病年龄、临床分型、吞咽困难、咳嗽无力、肌无力危象、糖皮质激素用量、辅助通气情况等与患者住院期间发生肺部感染关系密切。多因素回归分析结果显示,发病年龄、咳嗽无力、糖皮质激素用量、糖皮质激素疗程、新斯的明用量、阿托品用量、辅助通气情况等与患者住院期间发生肺部感染关系密切。结论引起重症肌无力住院患者发生肺部感染的影响因素较多。患者发病年龄、咳嗽无力、激素疗程长、剂量大,阿托品大量使用以及辅助通气措施等均为危险因素。临床应根据各种危险因素采取相应措施预防重症肌无力患者住院期间肺部感染的发生。OBJECTIVE To investigate the influencing factors of pulmonary infection of hospitalized patients with myasthenia gravis （MG）. METHODS The clinical data for 193 cases of MG patients from January, 2006 to April, 2012 were collected. The influencing factors of lung infection of hospitalized patients were analyzed. RESULTS There were 27 cases （17.53%） with pulmonary infection in the 193 cases of MG hospitalized patients. The univariate analysis showed that age, age at onset, clinical type, difficulty swallowing, cough, weakness, myasthenic crisis, glueocorticoid dosage, assisted ventilation were closely re- lated with the pulmonary infection of hospitalized patients with MG. Multivariate logistic regression analysis showed that age of onset, cough, weakness, glucocorticoid dosage, glucocorticoid treatment, neostigmine dosage, atropine dosage, assisted ventilation situation were closely related with the pulmonary infection of hospitalized patients with MG. CONCLUSION There are many risk factors leading to pulmonary infection of hospitalized patients with MG. The age of onset, cough, weakness, long hormone treatment, the dose of atropine extensive use of assisted ventilation measures are the risk factors. It＇s necessary to take appropriate measures based on the variety of risk factors to prevent the occurrence of pulmonary infection during hospitalization of patients with MG.
