临床护理路径在预防住院老年患者跌倒中的应用

摘　　要：目的:探讨临床护理路径管理模式在预防住院老年患者跌倒管理中的实施效果。方法:应用"住院患者跌倒危险因素评估、监控表"对60岁以上的住院患者进行跌倒危险性评估,筛选出跌倒高危患者205例,采用临床护理路径管理模式对患者进行护理干预,防止住院期间意外跌倒。结果:205例跌倒高危患者在住院期间发生跌倒1例,发生率为0.49%,跌倒率由2011年的2.15%下降到0.49%。结论:采取临床护理路径管理模式进行护理干预,能有效预防老年患者意外跌倒的发生。Objective ：To study the effect of the clinical nursing path for the prevention of falls in hospitalized elderly patients. Methods： To assess the falls risk in hospitalized patients over 60 years of age with applying the falls risk assessment table in hospitalized patients ,and filtered out 205 cases high -risk patients from falling with nursing intervention, prevented accidental fall during hospitalization. Results：The only one case fell in the 205 high -risk patients during hospitalization ,the incidence rate was 0.49 percent,and no one had serious consequences ,falling rate dropped to 0.49% from 2.15% in 201l. Conclusions ： It can effectively prevent the occurrence of the falls in hospitalized elderly patients with applying the clinical nursing path.
