摘　　要：目的　研究COX 1、COX 2、iNOS于浅表性胃炎、胃粘膜不典型增生及胃癌组织中表达的意义 ,探讨COX 2 ,iNOS与胃癌、幽门螺杆菌 (Hp)感染的关系。方法　应用多克隆抗体COX 1、COX 2、iNOS免疫组化方法检测 33例浅表性胃炎、30例胃粘膜不典型增生、2 9例胃癌石蜡包埋组织中的COX 1,COX 2 ,iNOS表达情况及Hp感染率。 结果　COX 2 ,iNOS在胃癌中的表达率明显高于浅表性胃炎(6 5 .5 % ,6 2 .1%比 18.2 % ,18.2 % ;P <0 .0 1)。Hp阳性的胃炎组织中COX 2 ,iNOS表达强度高于Hp阳性的胃粘膜不典型增生 ,COX 2、iNOS在浅表性胃炎、胃粘膜不典型增生及胃癌中同时表达。结论　COX 2 ,iNOS两酶的激活物可能相类似。COX 2 ,iNOS于胃癌中表达明显上调 ,两种酶参与肿瘤的形成 ,可能为胃肠道肿瘤形成的早期事件 ,胃粘膜炎症及Hp感染诱导COX 2、iNOS表达 。Objective To investigate the significance of expression of COX 1,COX 2 and iNOS in superficial gastritis, gastric mucosal atypical hyperplasia and gastric carcinoma and to identify the relationship among COX 2, iNOS, gastric carcinoma and Hp infection. Methods Immunohistochemical methods were used with the polyclonal antibody to COX 1, COX 2 and iNOS. The expression of iNOS, COX 1, COX 2 and the status of Hp infection in 92 cases of paraffin embedded gastric tissues were examined. Of the 92 patients, 33 had superficial gastritis, 30 gastric mucosal atypical hyperplasia and 29 gastric cancer. Hp was detected by Toluidine blue stain. Results Expression of COX 2 and iNOS in gastric cancer was significantly higher than that in gastritis (65.5%,62.1%vs 18.2%, 18.2%, P <0.01). Expression of COX 2 and iNOS in gastritis with Hp infection was higher than that in gastric mucosal atypical hyperplasia with Hp infection. The expression of COX 2 and iNOS was simultaneous in gastritis, gastric mucosal atypical hyperplasia and gastric cancer. Conclusion Inflammation and Hp infection could stimulate the expression of COX 2 and iNOS, which might promote gastric carcinogenesis.
