摘  要:目的比较颅内、颅外、颅内外动脉狭窄与肾动脉狭窄的关系。方法对516例缺血性脑血管病(ischemic cerebrovascular disease,ICVD)患者行数字减影血管造影(digital subtraction angiography,DSA)及肾动脉造影,根据DSA结果将患者分为正常组、颅内动脉狭窄组、颅外动脉狭窄组、颅内外动脉均狭窄组,各狭窄组又分为轻、中、重度及单支、双支、三支狭窄亚组,比较各狭窄组的肾动脉狭窄率。结果ICVD患者中肾动脉狭窄发生率为9.3%(48/516),其中正常组、颅内、颅外、颅内外动脉狭窄组的肾动脉狭窄发生率分别为3.7%、4.2%、12.57%、19.54%,其中重度颅外、颅内外动脉狭窄组及颅外动脉双支、三支以上狭窄组、颅内外动脉双支、三支以上狭窄组的肾动脉狭窄检出率分别为17.48%、21.79%、20.41%、25%、17.65%、20.75%,显著高于正常组及颅内动脉狭窄组(P<0.05),颅内动脉轻、中、重度狭窄亚组及单支、双支、三支以上狭窄亚组的肾动脉狭窄发生率分别为0、4.55%、4.76%、4.35%、4.17%、0,颅内动脉的狭窄程度及支数均与肾动脉狭窄无关联(P>0.05)。结论ICVD患者中,肾动脉狭窄多见于重度颅外、颅内外动脉狭窄及双支以上颅外、颅内外动脉狭窄患者。Objective To study the atherosclerotic renal artery stenosis (ARAS) of extracranial ,intracerebral and ex- tracranial-intracerebral artery stenosis in patients with ischemic cerebrovascular disease. Methods Data from 516 consec- utive patients with ischemic cerebrovascular disease (ICVD) who underwent simultaneous renal angiography following Dig- ital Subtraction Angiography (DSA) was collected prospectively. The patients with ICVD were divided into normal group, extracranial artery stenosis group, intracerebral artery stenosis group and extracranial-intracerebral artery stenosis group. Each stenosis group were further divided into mild, moderate and,severe artery stenosis groups based on the severity and 1 branch, 2 branchs and 3 branchs stenosis groups according to branch involved. Results ARAS was found in 48(9.3% )of patients with ICVD. Among them, the rates of ARAS were 3.7 %,4.2%,12.57% and 19.54% in the normal group, extracra- nial artery stenosis group, intracerebral artery stenosis group and extracranial-intracerebral artery stenosis, respectively. In the subgroup, the rates of ARAS were 17.d8%, 21.79%, 20.41%, 25%,17.65% and 20.75% in severe extracranial, extra- cranial-intracerebral, 2 branch- and 3 branch- extracranial and extracranial-intracerebral artery stenosis groups, respec- tively. The rates of ARAS were 0, 4.55%, 4.76%, 4.35%, 4.17% and 0 in mild, moderate, severe ,lbranch-, 2 branch- and 3 branch- intracerebral artery stenosis groups, respectively. Conclusions ARAS are common in patients with ICVD, par- ticulalry in those with severe extracranial ,extracranial-intracerebral artery stenosis and the 2 branch-, 3 branch-extracra nial and extracranial-intracerebral artery stenosis.
