地佐辛联合帕瑞昔布钠对食管癌根治术患者全麻苏醒期躁动的疗效观察

摘　　要：目的评价地佐辛联合帕瑞昔布钠对食管癌根治术患者全麻苏醒期躁动的疗效。方法选择全身麻醉下行食管癌根治术患者80例(ASAⅠ或Ⅱ级),随机分为四组,每组各20例,术毕前30 min分别静脉注射地佐辛10 mg(A组)、帕瑞昔布钠40 mg(B组)、地佐辛10 mg+帕瑞昔布钠40 mg(C组)及等容量生理盐水(D组)。比较四组苏醒期躁动的发生率、苏醒及意识恢复时间、镇痛及镇静评分情况。结果 A、B、C三组的躁动评分、躁动发生率和镇痛评分明显低于D组(P<0.05),而C组又明显低于A组和B组(P<0.05);B、D组的镇静评分高于A、C组(P<0.05);四组均无明显不良反应。结论地佐辛联合帕瑞昔布钠可有效预防或减少食管癌根治术患者全麻苏醒期躁动,值得临床推广。Objective To evaluate the effect of Dezocine combined with Parecoxib Sodium on emergence agitation in patients of radical operation for esophageal cancer. Methods 80 patients （ASA I or II ） undergoing general anesthesi- a for esophageal carcinoma were randomly assigned to 4 groups, with 20 cases in each group： Dezoeine 10 mg （group A）; Pareeoxib Sodium 40 mg （group B）; Dezocine 10 mg plus Parecoxib Sodium 40 nag （group C）; euvolemia normal saline （group D）. All the patients were administered intravenously different group drugs 30 minutes before the end of surgery. The occurrence of emergence agitation, waking up and consciousness recovery time, analgesia and sedation scores were observed. Results The agitation scores and incidence of agitation and analgesia scores were significantly lower in group A, B, C than those in group D （P 〈 0.05）, but group A and B were higher than those of group C （P 〈 0.05）. The sedation scores of group B and D were higher than those of group A and C （P 〈 0.05）. There were no sig- nificant differences between 4 groups in awake and consciousness time during emergence from general anesthesia. Conclusion Dezocine combined with Parecoxib Sodium intramuscular injection can significantly reduce the incidence of agitation effectively and safely during emergence from general anesthesia in patients of esophageal carcinoma under- going radical operation, which is worthy of clinical promotion.
