70例食管癌病人的FDG摄取临床影响因素分析

摘　　要：目的：分析并总结不同因素对食管癌原发灶FDG摄取所产生的影响。方法：将我院自2009年12月~2011年6月期间收治的实施18F-FDG PET/CT全身扫描的病人70例,结合术后病理检查结果分析其病变长度、淋巴结转移以及浸润深度状况,比较不同因素对食管癌原发灶FDG摄取所产生的影响,并分析转移状态、TNM分期与SUVmax值之间的相关性。结果：女性组的SUVmax低于男性组,两组相比未见显著差异（P=0.382）,不具有统计学意义。从不同年龄段角度进行对比,高龄组病人的FDG摄取量比较高,但三组病人间的SUVmax比较无明显差异（P=0.162）,不具有统计学意义;FDG摄取与病变长度成正比关系（P=0.000）;相关性分析结果显示病变长度与原发灶SUVmax呈正相关关系（P=0.01）;T1、T2、T3三组之间原发灶的SUVmax存在明显差异（P=0.017）;伴随病人的浸润深度增加,其SUVmax值也出现逐渐升高的趋势。病人浸润深度同原发灶SUVmax之间具有正相关关系（P=0.000）;分析转移状态与FDG摄取的相关性结果显示SUVmax值越大,则转移程度就越高（P=0.000）。结论：食管癌原发灶FDG摄取与病人的年龄及性别无相关性,而与病变长度及TNM分期等具有密切的相关性,SUVmax值较高表明已经出现淋巴结转移或TNM分期较晚。Objective： To analyze and summarize the different factors in primary esophageal carcinoma FDG uptake of the impact.Methods： In our hospital from 2009December to2011June were the implementation of 18F-FDG PET/CT body scans of 70 patients,with postoperative pathological examination results analysis of the lesion length,lymph node metastasis and invasion depth condition,comparison of different factors on primary esophageal carcinoma FDG uptake by the impact,and the analysis,transfer status TNM staging and the correlation between SUVmax value.Results： the female group SUVmax was lower than the male group,two groups but there were no significant difference（P=0.382）,were not significant.From different ages angle undertakes contrast,elder patients in group FDG intake is relatively high,but the three groups has no significant difference of SUVmax（P=0.162）,were not significant;FDG uptake and lesion length proportional to the relationship（P=0）;the results of correlation analysis showed the lesion length and SUVmax in primary foci a positive correlation（P=0.01）;T1,T2,T3 between the three groups of primary SUVmax obvious difference（P=0.017）;with patient infiltration depth increases,the value of SUVmax also appear increasing trend.Patients with primary focal depth of invasion of SUVmax had a positive correlation（P=0）;analysis of metastasis and FDG uptake correlated the result shows that the SUVmax value is greater,the transfer rate is higher（P=0）.Conclusion： Primary esophageal carcinoma FDG uptake and the patient＇s age and sex no correlation,and lesion length and TNM stage is closely related to higher SUV max,that has the appearance of lymph node metastases or TNM staging later.
