甘露聚糖肽对Ⅳ期卵巢癌治疗作用的临床研究

摘　　要：目的:探讨甘露聚糖肽序贯疗法联合同步放化疗治疗对IV期卵巢癌的治疗作用。方法:选择162例IV期卵巢癌患者,随机分为试验组81例和对照组81例,试验组使用甘露聚糖肽序贯治疗结合多西他赛/卡铂方案化疗,对照组使用胸腺肽序贯治疗结合多西他赛/卡铂方案化疗,治疗结束后观察两组的T淋巴细胞亚群(CD3+,CD4+,CD8+)和NK细胞百分率、外周血白细胞、抗体(IgA、IgG、IgM)、补体(C3)、中性粒细胞等免疫学指标的变化并随访观察5年,1年后进行卡氏评分,并计算其中位生存期。结果:试验组与对照组比较,各免疫指标提升明显(P<0.05),尤其是NK细胞水平及中性粒细胞水平明显(P<0.01);而胃肠反应(P<0.01)、肝功能损害(P<0.01)、贫血(P<0.01)、肾功能损害(P<0.05)、脱发(P<0.05)等不良反应较对照组轻;试验组患者的卡氏评分和中位生存期均有提高,且与对照组相比有显著性差异(P<0.05)。结论:甘露聚糖肽序贯治疗与放化疗联合治疗晚期卵巢癌,可显著提高患者生存率,全面增强患者免疫功能,减低放化疗带来的副作用,效果优于胸腺肽序贯治疗。Objective：To investigate the therapeutic effect of the mannatide sequential therapy joint synchronous radiotherapy and chemotherapy in the treatment of stage IV ovarian cancer.Methods：162 patients with stage IV ovarian cancer were randomly divided into a test group and a control group,which consisted 81 cases respectively.The test group was treated with mannatide sequential therapy combined with docetaxel / carboplatin chemotherapy,and control group with thymus peptide sequential therapy combined with docetaxel / carboplatin chemotherapy.After treatment,T lymphocyte sub-group（CD3＋,CD4 ＋,CD8 ＋） and NK cell percentage,peripheral blood leukocyte,antibodies（IgA,IgG,IgM）,complement body（C3）,neutrophils and other Immunological index were observed,and observations were lasted 5-year follow-up.After one year,the Karnofsky score was carried out,and then the median survival period was calculated.Results：Compared with the control group,immune indicators of experimental group were improved significantly（P0.05）,especially NK cells and neutrophils were significant（P0.01）;Gastrointestinal effects（P0.01）,liver dysfunction（P0.01）,anemia（P0.01）,renal damage（P0.05）,hair loss（P0.05） and other adverse reactions in test group were lighter than that in the control group;Karnofsky score and the median survival time of test group patients were improved,They had a significant difference compared with the control group（P0.05）.Conclusion：Mannatide sequential treatment with chemotherapy combined treatment of advanced ovarian cancer can significantly increase the survival rate of patients,comprehensively enhance the immune function of patients,and reduce the side effects of radiotherapy and chemotherapy,better than thymus peptide sequential therapy.
