三种无创性心脏检测在犬小灶心肌梗死中的应用比较

摘  要:目的 :比较犬心肌陈旧性梗死时三种无创性检测法参数的动态变化及特点 ,初步评价其诊断价值。方法 :结扎冠状动脉左前降支远端二级分支 ,建立犬陈旧性局灶性心肌梗死模型 ,观察心脏收缩时间间期 (使用 STI) ,高频心电图 (HFECG)和心肌灌注显像 (使用 SPECT)的变化特点。结果 :陈旧性局灶性心肌梗死后 ,STI的各项指标虽有改变 ,但不显著。HFECG的切迹数在 2~ 4周时明显增多 ,而在 8周时恢复 ;在 2~ 4周时 ,SPECT显示心肌前壁梗死区呈放射性稀疏及缺损 ,而在 8周时 ,该异常明显恢复。结论 :STI对局灶性心肌梗死不敏感 ,HFECG对局灶性心肌梗死较敏感 ,可以反映缺血性心脏病的严重程度 ,而 SPECT能直观的反映心肌缺血和心肌梗死 ,是三者中最灵敏的检测方法。AIM:According to the comparison of dynamical data and character of three noninvasive cardiac detection methods in canine myocardial infarction model, the diagnostic value was basically evaluated. METHODS:The remote regional myocardial infarction model was founded after the distal and second level branch of canine left anterior descending coronary artery was ligated. Changes in noninvasive cardiac detection such as systolic time interval (STI),high frequency electrocardiogram (HFECG) and myocardial imaging (by SPECT) were observed. RESULTS:After remote regional myocardial infarction,the parameters of STI did not significantly change; the total notching number in HFECG increased markedly within 2~4 weeks and recovered at 8th week;the radial rarefaction and defect were indicated in infarction area of anterior wall of myocardium with 2~4 weeks in SPECT, and these abnormalities ameliorated obviously at 8th week. CONCLUSION:STI was not sensitive to regional myocardial infarction. HFECG was relatively sensitive to regional myocardial infarction,it could reflect the severe extent of ischemic heart disease. SPECT could reflect myocardial ischemia and myocardial infarction,and it was the most sensitive method.
