颅脑彩超在脑性瘫痪患儿脑白质损伤中的应用价值探讨

摘　　要：目的探讨颅脑彩超在脑性瘫痪患儿脑白质损伤中的应用价值。方法将本院小儿神经康复科2011年1月-2012年12月收治的90例脑性瘫痪患儿分成A、B两组，所有患儿均给予头针治疗，通过对患儿分别采用颅脑彩超、MRI检查（外院）检查，比较不同检查方法在脑瘫患儿的脑白质损伤中的应用价值。结果A组治疗前Ⅰ级损伤患儿10例，占22．2％，Ⅱ级损伤10例，占22．2％，轻度损伤患儿共20例，占44．4％；治疗后Ⅰ级损伤患儿13例，占28．9％，Ⅱ级损伤17例，占37．8％，轻度损伤患儿共30例，占66．7％，治疗前后A组患儿的颅脑彩超结果与临床脑瘫患儿活动受限程度分级匹配效果较好，B组患儿治疗前轻度损伤者18例，占40．0％，中度损伤14例，占31．1％，重度损伤13例，占28．9％，治疗后轻度损伤者29例，占64．4％，中度损伤者9例，占20．0％，重度损伤者7例，占15．6％，治疗前后B组患儿的MRI检查结果与临床颅脑瘫痪患儿的活动受限程度符合度较高。结论颅脑彩超能够动态的观察脑瘫患儿的脑向质损伤情况，较MRI检查经济、方便、安全，具有较高的临床应用价值。Objective To investigate the in application value of brain uhrasonography in children with cerebral palsy cerebral white matter injury. Methods 90 cases cerebral palsy children who were treated in the department of neurological rehabilitation od our hospital from 2011 january to 2012 December were chosen as the research object,then theny were divided into group A and group B,all patients given scalp acupuncture therapy,the application value in cerebral palsy children with cerebral white matter injury were compared by head color Doppler ultrasound,and MRI（outer court）. Results 10 cases were Ⅰ injury in group A before treatment,it accounted for 22.2%,10 cases were Ⅱ injury,it accounted for 22.2%,20 cases were mild injury,it accounted for 44.4%,but 13 cases were Ⅰ injury in group A before treatment,it accounted for 28.9%,17 cases were Ⅱ injury,it accounted for 37.8%,30 cases were mild injury,it accounted for 66.7%,the head ultrasound results and clinical manifestations in children with cerebral palsy activity limitation grading matching effect is good before and after treatment in group A,18 cases were mild injury in group B before treatment,it accounted for 40.0%,14 cases were moderate injury,it accounted for 31.1%,13 cases were severe injury,it accounted for 28.9%,but 29 cases were mild injury in group B before treatment,it accounted for 64.4%,9 cases were moderate injury,it accounted for 20.0%,7 cases were severe injury,it accounted for 15.6%,the brain MRI results and clinical manifestations in children with cerebral palsy activity limitation grading matching effect is good before and after treatment in group B. Conclusion The eraniocerebral color uhrasonic can dynamically observe the cerebral palsy children with cerebral white matter damage repair,it is economic, convenient and safe compared to MRI,it is high value in clinical application.
