摘 要:目的探讨肝细胞癌(HCC)、胆管癌、肝硬化及正常肝组织中增殖细胞核抗原(Ki-67)、血管内皮生长因子(VEGF)及血管内皮生长因子受体(Flt-1)蛋白表达的意义及其与肝癌生物学行为的关系。方法对52例符合要求的HCC组织、10例胆管癌组织、10例肝硬化组织及10例正常肝组织采用免疫组化S-P法检测Ki-67、VEGF及Flt-1蛋白。结果 Ki-67、VEGF及Flt-1在HCC的强阳性表达率分别为48.1%、55.8%及67.3%,均显著高于胆管癌、肝硬化及正常肝组织(P<0.001)。Ki-67与肿瘤大小(P<0.001)、血管侵犯(P<0.05)、分化程度(P<0.05)有关;VEGF表达与肿瘤包膜完整、血管侵犯及肿瘤分化程度明显有关(P<0.05);Flt-1与肿瘤分化程度明显有关(P<0.05)。Ki-67与VEGF及Flt-1间无相关(P>0.05)。VEGF与Flt-1间的蛋白表达呈正相(R=0.332,P<0.01)。结论 Ki-67、VEGF及Flt-1蛋白的过表达可促使肝癌细胞增殖,使肝癌细胞具有更强的侵袭力,与肝癌的发生、发展密切相关。[ Objective] To evaluate the correlation between the expression of Ki-67, VEGF and Fh-1 genes and the biological behavior of hepatocellular carcinoma (HCC). [Methods] Immunohistochemical technology was applied to detect the expression of Ki-67, VEGF and Fh-1 in 52 HCC tissues and their adjacent liver tissues. [Results] The over expression of positive rates of Ki-67 and VEGF and Fh-1 were 48.1%, 55.8% and 67.3% respectively, and significantly higher in the HCC tissue than those of adjacent liver tissue (P 〈0.001). The over expression of posi- tive rates of Ki-67 in the HCC tissue were significantly correlated with the tumor size (P 〈0.001), presence of extra- hepatic metastasis, and the differentiation of tumor (P 〈0.05). The over expression of positive rates of VEGF in the HCC tissue were significantly correlated with the tumor capsula, presence of extrahepatie metastasis, and the differ- entiation of tumor (P 〈0.05). The positive rate of Fh-1 was significantly correlated with the differentiation of tumor (P 〈0.05). There was a positive correlation between the expression of VEGFand Flt-1 in the HCC tissue (R =0.332, P 〈0.01). [ Conclusion] The over- expression of Ki-67, VEGF and Fh-1 proteins may contribute to the prolifera- tion of hepatoma cell, improve the aggressive ability of hepatoma cell, and relates to the initiation and progression of HCC.
