老年患者胃和结、直肠癌根治术后5′-DFUR化疗与5-FU联合化疗(附100例分析)

摘　　要：目的　探讨 5′- DFU R(氟铁龙 )化疗方案在胃和结、直肠癌根治术后老年患者中的应用 ,寻找更符合这类患者的术后化疗方法。方法　选择了自 1994年 1月～ 1998年12月间 5 0例胃和结、直肠癌根治术后应用 5′- DFU R化疗的老年患者与 5 0例应用 5 - FU联合化疗的老年患者 ,对两组术后化疗方案进行回顾性的对比。比较项目包括 :患者对化疗药物的毒副反应 ;肿瘤的转移和复发 ;化疗后并发症 ;生存率 ;生活质量测评 ;平均住院日及费用等。结果　患者对化疗药物的毒副反应 ;化疗并发症和生活质量测评等项目 ,5′-DFU R组优于 5 - FU联合化疗组。而肿瘤的转移、复发和生存率两组比较无明显差别 ;平均住院日 5′- DFU R组低于 5 -FU联合化疗组 ;而平均医疗费用 5′- DFUR组并不高于 5 -FU联合化疗组。结论　 5′- DFU R在治疗胃和结、直肠癌根治术后的老年患者中 ,综合效果优于 5 -Objective To evaluate retrospectively the effets of 2 different methods of postoperative adjuvent chemotherapy, single use of 5′ DFUR and combined use of 5 FU, for stomach and colo rectal cancers of elderly patients. Methods 2 groups of stomach and colo rectal cancers of elderly patients,each group 50 cases, treated in 1994,1998,12,entered this study. One group received single 5′ DFUR and another group received 5 FU+MeCCNU,5 FU+MMC,5 FU+DDP or 5 FU+MMC+ADM.Both groups of patients received Calcium Tetrahydrofolate (CF) simultaneously. The toxicty and side effects of chemotherapy, complications, tumor metastasis and recurrence, survival rates, time of hospitalization, medical expenses of patients were analyzed and compared. Results The toxicity and side effects,complications and performance states were better in 5′ DFUR group. Tumor metastasis,reccurrence and survival rates of two groups showed no significant different. The time of hospitalization was shorter and the medical expenses were higher in 5′ DFUR group.Conclusion Single 5′ DFUR treatment may be a better postoperative adjuvant chemotherapy for stomach and colo rectal cancers of elderly patients.
