肠外营养对胃癌患者化疗期间免疫功能的影响研究

摘　　要：目的：研究肠外营养对胃癌患者化疗期间免疫功能的影响。方法：选取2009年6月~2011年6月我院的100例胃癌患者为研究对象,随机分为2组,肠外营养组50例给予全肠外营养治疗,肠内营养组50例化疗期间给予肠内营养,化疗开始后第1天开始行肠外或肠内营养支持,持续8天。检测并记录化疗前和化疗后第8天患者的细胞免疫指标CD3、CD4、CD8和NKC活性及体液免疫指标IgA、IgG、IgM。结果：肠外营养组化疗后CD3、CD4、CD8值及NKC活性明显下降,IgA、IgG、IgM值明显升高;肠内营养组化疗后CD3、CD4、CD8水平和NKC活性与化疗前相比无显著性变化,IgA、IgG、IgM水平也明显升高。结论：肠外营养对胃癌病人化疗后细胞免疫功能无明显改善作用,肠内营养有明显的细胞免疫增强作用,可有效地纠正胃癌病人化疗后细胞免疫抑制状态。Objective：To investigate the influence of parenteral nutrition on gastric cancer patients, immune function during chemotherapy. Methods：100 cases with gastric cancer were chosen as research objects from Jun. 2009 to Jun. 2011, they were randomly divided into two groups, 50 cases of patenteral nutrition group were given total parenteral nutrition treatment, and 50 cases of enteral group were given enteral nutrition treatment. 1 day after the start of chemotherapy, parenteral and enteral nutrition supports were given, and for 8 days. The cellular immune indicators CD3、CD4、CD8, NKC activity and humoral immunity IgA、IgG、IgM were detected and recorded before chemotherapy and the eighth day after chemotherapy. Results：CD3、CD4、CD8 values and NKC activity of the patenteral nutrition group after thermotherapy were obviously declined, IgA、IgG、IgM values were obviously increased; while CD3、CD4、CD8 levels and NKC activity of the enteral group had no significant differences before and after chemotherapy, IgA、IgG、 IgM levels were also significantly increased. Conclusions： Paternternal nutrition had no significantly improvement role to gastric cancer patients, enteral nutrition could obviously increase the cytoimmunity role, and could effectively correct the patients, cell immunosuppresion after chemotherapy.
