摘　　要：目的　生长在脑内和脑外的海绵状血管瘤虽然名称相同 ,但临床表现、手术难度和术后效果差别甚大。它们可能存在着不同的组织学亚型 ,本文试图对此种肿瘤进行进一步分类。方法　对我院 1981年以来经手术治疗的 2 5例颅内海绵状血管瘤进行回顾分析。结果　根据病人的临床表现、术中所见和病理检查将 2 5例病人分为 2类 3型 :Ⅰ型 :病变位于脑内 ,15例 ,均全切除肿瘤 ,术后症状改善 ;Ⅱa型 :5例 ,病变位于脑外 ,肿瘤切除极其困难。Ⅱb型 :脑外病变 ,5例 ,肿瘤含较多实质成分 ,能够完全切除。Objective Although there are the identical denomination for the intra axial and extra axial cavernous hemangiomas,there is much different between their clinical presentation,operative difficulty and prognosis.The purpose of this paper is to divide these tumors into different subgroups according to the clinical manifestations,intraoperative findings and pathological results. Methods twenty five patients with intracranial cavernous hemangiomas treated surgically from May 1981 to August 1997 in our hospital were analyzed were retrospectively. Results 25 patients were divided into 2 groups.Group I represents those cavernous hemangiomas located intra axially with the symptoms of headache,epilepsy could tumor bleeding.In this group,total removal of the tumors and the excellent postoperative results could be expected(n=15).Group II pointed to the tumors located extra axially with the presentation of headache and the dysfunction of the cranial nerves. This group was further divided into 2 subgroups designated as group IIa and Group IIb.Group IIa represents the patiented with intraoperative findings of the bluish red,very soft and visible pulsation of the tumor and the high risk of excessive bleeding(n=5).It was extremely difficult or even impossible to remove these tumors, completely.The tumors of group IIb appeared to be encapsulated with a layer of the pseudocapsule,and could be removed totally using the microneurosurgical techniques(n=5).Conclusions Detailed classification of the intracranial cavernous hemangiomas and management intraoperative.
