摘 要:目的:评价血必净注射液对重症急性胰腺炎(SAP)大鼠肠黏膜屏障损伤的实验疗效。方法:30只雌性SD大鼠被随机分成假手术组(S组)、对照组(D组)、血必净治疗组(T组),后两组经胰胆管逆行注射4.5%牛磺胆酸钠制备大鼠SAP模型,T组于建模后3 h经鼠尾静脉注射血必净注射液(3 mg/kg);建模后24 h处死大鼠,检测各组血清淀粉酶(AMS)、肿瘤坏死因子α(TNF-α)、内皮素(ET)、二胺氧化酶(DAO)和D-乳酸水平,并对小肠组织病理进行评分。结果:S组、D组、T组血清AMS分别为(224.8±51.7)、(2374.5±331.3)、(924.2±281.8)U/L,S组、D组、T组血浆TNF-α水平为(14.9±3.8)、(154.6±12.9)、(95.2±15.9)U/L,ET水平分别为(21.2±14.4)、(82.6±19.4)、(69.8±11.2)ng/L,3组比较,差异均有统计学意义(均P<0.05);DAO水平分别为(35.8±14.5)、(176.3±13.9)、(115.1±12.8)ng/mL,D-乳酸水平分别为(1.2±0.4)、(2.6±0.4)、(2.0±0.2)mmol/L,3组比较,差异均有统计学意义(均P<0.05);S组、D组、T组大鼠肠道病理评分分别为(2.3±0.4)、(6.7±0.6)、(4.2±0.8)分,3组比较,差异有统计学意义(P<0.05)。Objective: To evaluate the effects of Xuebijing injection ( XBJ ) on the injury of intestinal mucosal barrier in rats with severe acute pancreatitis ( SAP ). Methods: 30 female SD rats were randomly divided into namely sham group (S group), control group (D group) and XBJ treatment group (T group) among which the latter two groups were injected 4.5% sodium taurocholate through into the pancreatic duct by retro for establishment of SAP rat models. Notably, T group was intravenously injected 3 mg/kg XBJ througb tails at 3h after modeling. The rats were sacrificed at 24h after modeling and detected the levels of amylase (AMS) , tumor necrosis factor-α (TNF-α), endothelin (ET), diamine oxidase (DAO) and D - lactic acid and performed scoring on their pathological tissue slices of small intestine. Results: In S, D and T groups, the levels of AMS were (224.8 ± 51.7 )U/L, (2374.5 ± 331.3) U/L and (924.2 ± 281.8 )U/L, the levels of TNF-α were ( 14. 9 ± 3.8) U/L, ( 154.6 ± 12.9) U/L and (95.2 ± 15.9 ) U/L and the levels of endothelin were (21.2 ± 14.4)ng/L, (82.6±19.4) ng/L and (69.8 ±11.2) ng/L (all P〈0.05). In S, D and T groups, the levels of DAO were (35.8 ± 14.5) ng/mL, ( 176.3 ± 13.9) ng/mL and ( 115.1 ± 12.8 ) ng/mL, and the levels of D- Lactate were ( 1.2 ± 0.4) nmol/L, (2.6 ± 0.4) nmol/L and (2.0 ± 0.2) nmol/L ( all P 〈 0.05 ). In S, D and T groups, the Intestinal pathological scores were (2.3 ± 0.4 ) , ( 6.7 ± 0.6) and (4.2 ± 0.8 ) ( P 〈 0.05 ). Conclusion: The early application of XBJ could alleviate the injury of intestinal mucosal barrier in SAP rats.
