摘　　要：目的:为选择最优的鼻咽癌同步加量调强放疗计划建立一个新的计划质量评估指标。方法:自行编写一个Matlab程序读取TPS中的DVH数据。计算各计划质量指标并建立计划过滤矩阵。结合剂量的和生物计划质量指标建立新的综合计划质量评估指标(CPQI)用于评估和比较鼻咽癌同步加量调强计划。并通过详细的统计分析和医生的评估结果对该新的评估指标进行验证。结果:对于不同危急器官(OAR)保护权重的计划,OAR low,OAR normal,OAR high和PTV only计划,CPQI的值分别为0.22±0.08,0.49±0.077,0.71±0.062,-0.21±0.16。这些计划之间的计划质量指标有显著的差异。详细的统计分析验证了该指标的准确性,并与医生的评估选择结果一致。结论:新的计划质量评估指标准确简便,可以为鼻咽癌病人选择最优的同步加量调强计划提供帮助。Objective： To propose a new plan quality index for simultaneous integrated boost intensity modulated radiotherapy （SIB-IMRT） in the treatment of nasopharyngeal cancer. Methods： Ten nasopharyngeal cancer patients treated by SIB-IMRT were enrolled in the study. Custom software was written to read cumulative and differential DVH curves from the treatment planning system （TPS）. A plan filtering matrix with items of plan quality indices was introduced to filter plans that fail to satisfi） treatment protocol. A unique composite plan quality index （CPQI） was proposed based on the relative weight of plan quality indices to evaluate and compare plans. Plan ranking results were compared with detailed statistical analysis, and physician＇s evaluation results to verify the accuracy of this new plan quality index. Results： The average CPQI values for plans with OAR priority of low, normal, high, and PTV only were 0.22±0.08, 0.49±0.077, 0.71 ±0.062, and -0.21±0.16, respectively. There were significant differences between these plan quality indices （One-way ANOVA test, p〈0.01）. This was consistent with statistical analysis and physician＇s ranking results in which 90 % OAR high plans were selected. Conclusions： Plan filtering matrix was able to speed up the plan evaluation process. The new matrix plan quality index CPQI showed good consistence with physician ranking results. It is a promising index for radiotherapy plan evaluation.
