摘　　要：目的 探讨破裂脑动脉瘤64层螺旋CT血管造影三维重建（3D-MSCTA ）的表现，提高对破裂脑动脉瘤的认识。方法 对100例破裂和90例未破裂脑动脉瘤患者的MSCTA表现和手术所见进行对照分析，所有影像图像均由高年资影像科医师和神经外科医师共同阅读。结果 破裂脑动脉瘤与未破裂脑动脉瘤组间差异有统计学意义（χ2=132.698，P＜0.001）。瘤体的不同影像征象与瘤体的部位之间差异无统计学意义（χ2=3.46，P＞0.05）。结论 脑动脉瘤表面征象的前4种征象，即乳头征、丘状突起征、棘突征或乳头＋棘突征，其中任何一种征象出现即可提示该瘤体已破裂，可前瞻性指导临床处置风险动脉瘤；光滑型征象提示瘤体未破裂出血。Objective To explore the imaging features of ruptured cerebral aneurysms on three-dimensional reconstruction of 64- slice spiral CT angiography（3D-MSCTA）, and to improve the understanding of the cerebral aneurysm rupture. Methods MSCTA performances and surgery findings for 100 cases with ruptured cerebral aneurysms and 90 cases with unruptured cerebral aneurysms were analyzed and compared,and all images were viewed by senior radiologists and neurosurgeons together. Results Ruptured cerebral aneurysms and unruptured cerebral aneurysms had statistically significant difference between groups （X^2= 132. 698, P〈0. 001）. The different signs of the aneuryms had no significant difference in different locations（X^2= 3.46,P〉0.05 ）. Conclusion Any one of the four kinds of signs, namely nipple sign, hill quilted sign, spinous process sign or papilla plus spinous process sign can indicate the rupture of the aneurysm：smooth type sign hint aneurysm unruptured bleeding.
