摘　　要：目的 探讨脾脏淋巴管瘤的影像表现，提高该病的诊断水平。方法 回顾性分析17例经手术病理证实的脾脏淋巴管瘤的临床及CT、MRI资料。结果 17例均发生于脾实质，4例合并有其他部位淋巴管瘤。脾内病灶单发5例，多发12例。CT平扫表现为脾实质内囊性低密度影，6例为单囊性病变，11例病灶内可见间隔。增强扫描15例囊壁轻度强化，间隔增强后静脉期轻～中度强化9例，延迟期轻度强化5例。MRI T1WI呈低或稍低信号，T2WI呈高信号，病灶内分隔呈低信号，增强扫描囊壁及间隔轻～中度强化。并发的纵隔、腹腔、腹膜后病变均为单囊，增强后未见囊壁强化；肝脏病变边界清楚，囊壁轻度强化；骨骼病变累及多骨，病灶边缘骨质轻度硬化，增强扫描病灶均未见强化。结论 脾脏淋巴管瘤的影像学表现具有一定的特征性，CT和MRI检查可准确显示病变的范围及数目，为临床诊断和治疗提供有价值的信息。Objective To investigate the imaging features of splenic lymphangioma, and improve the diagnosis of this disease. Methods Clinical data, CT and MRI of 17 cases with splenic lymphangioma confirmed by pathology were retrospective analyzed. Resuits All the lesions in the 17 cases were located in parenchyma of spleen, except 4 cases combined with lymphangioma in other organs. Five cases had single lesion in the spleen, while the other 12 cases showed multiple lesions. The performance of CT scan was presented as cystic low density in the splenic parenchyma, where in 6 cases were single-locular lesion, while the other 11 cases showed seen gap in the lesion. After contrast-enhancing, capsule walls of 15 eases showed enhancement slightly; gap within lesion was enhanced followed by venous phase gently or moderately in the 9 cases; and there were 5 cases showed enhancement at delayed period. On MRI, lesions showed low or slightly low singal on T2WI, while high signalon T2WI with low signal for gap within the lesion. Capsule walls and the gap showed mild to moderate enhancement after contrast ejection. Concurrent mediastinal, abdominal ,and retroperitoneal lesions were single capsule without enhancement of capsule wall. The borderline of lesions in liver was clear, and capsule wall was slightly enhanced. The skeleton lesion affected many bones, and sclerotin around the lesion was slightly hard- ened without enhancement. Conclusion CT and MR1 performances of splenic lymphangioma has certain characteristics, CT and MRI can accurately show the scope and number of lesions and provide valuable information for clinical diagnosis and treatment.
