摘　　要：目的 探讨妊娠滋养细胞疾病（gestational trophoblastic disease，GTD）在3.0T MRI的表现，以提高对本病的诊断水平。 方法 回顾性分析19例妊娠滋养细胞疾病的MRI表现、病理资料、手术所见及临床资料。结果 19例中，葡萄胎7例，MRI表现为子宫体积增大，宫腔内见“葡萄”状或“蜂窝”状肿块影，分隔较均匀，肿块包膜完整；增强6例分隔均匀强化，1例分隔强化欠均匀。侵袭性葡萄胎6例及绒毛膜癌6例，MRI显示其中11例宫体扩大，腔内“蜂窝”状肿块影，分隔杂乱；3例合并肿块内出血；10例包膜不完整，侵犯肌层；9例显示分隔及实性部分强化，2例肿块内见片状强化，似“血湖”状；11例子宫周围血管增多、迂曲；12例均显示肿块内部血管增粗、增多。结论 妊娠滋养细胞疾病的MRI具有较特征性表现，能较好地显示病变的内部结构、明确肿瘤范围，结合临床病史及实验室检查可以作出诊断，并可在早期提示病变的良恶性，从而指导临床制订治疗方案。Objective To explore the manifestations of gestational trophoblastic disease（GTDJ at 3.0T MRI,and to improve the diagnostic knowledge of GTD. Methods The MRI manifestations, pathologies,operational records and clinical data were analyzed retrospectively in 19 cases with GTD. Results Among 19 cases of GTD,7 cases were hydatidiform mole,with increased the uterine volume on MRI,a bump as ＂honeycomb＂ or ＂grape-like＂ in the cavity of the uterus, separated more uniform and complete tumor capsule. Separated homogeneous enhancement was seen in 6 cases,and 1 case no uniform enhanced. There were 6 cases of invasive hydatidiform mole and 6 cases choriocarcinonla, On MRI cavity expansion ,＂honeycomb＂ shape separated mix and disorderly were showed in 1 l cases ,bleeding in mass internal in 3 cases, capsule incomplete and infringement upon muscle layer in 1（1 cases, Both septal enhancement and the whole solid enhancement were observed in 9 cases, intra-mass enhancement like＂blood lake＂ in 2 eases. The uterine peripheral vascular was increased and tortuous in 11 cases, intra-mass vascular was increased and thickened in 12 cases. Conclusion GTD shows characteristic manifestations on MRL MRI can show the intra-lesions structures and the extent of the lesions very well,combined clinical history with laboratory examination,GTD can be diagnosed,and imply benign and malignant in the early lesions to guide treatment plan.
