正常大鼠部分脑组织切除术后术区周边脑组织DWI表现及其病理学基础

摘　　要：目的 研究正常大鼠脑组织部分切除后术区周边脑组织磁共振弥散加权成像（DWI）的特点及其病理学基础。方法 30只清洁级成年Sprague Dawley大鼠随机分为术后3 d、7 d、14 d、21 d组及正常对照组。制作大鼠部分脑组织切除模型，对照组不做任何处理。行鼠脑T2WI和弥散加权成像，观察术后不同时间点术区周边脑组织和对照组相应部位DWI的特点。MR检查完成后处死动物，观察相应时间点术区周边脑组织的病理学改变。结果 （1）影像学结果：手术各组术区周边脑组织ADC值大于对照组（P〈0.05），ADC值于术后7 d达最大值（P〈0.05）。（2）病理学结果：HE染色，术区周边脑组织见不同程度的炎症细胞浸润、星形胶质细胞增生、新生毛细血管和脑水肿。术后各组术区周边脑组织细胞密度大于对照组（P〈0.05）。结论 正常大鼠部分脑组织切除术后，术区周边脑组织DWI表现特点及其ADC值变化与术后时间具有相关性，其病理学基础为脑水肿、新生毛细血管增多和星形胶质细胞增生。Objective To observe the diffusion weighted imaging （ DWI ） features of surgical margin and its pathologic basis of the resetting partial brain tissue in experimental rat mode. Methods Thirty rats were randomly divided into four experimental groups 3 d,7 d, 14 d,21 d groups postoperatively,and one control group which did not undergo operation. A partial resection of the parietooccipitaI region in all rats was performed. MR imaging procedures consisted of T2WI and DWI. Brain tissue tissue samples were obtained after MR imaging and the pathological features were observed. Results （ 1 ）Imaging results：the ADC value in the resecting margins of the brain was higher than that in control group and reached summit at 7 d postoperatively（P〈0.05 ）. （2 ） Pathologic results ：①HE showed different extent of inflammatory cell infiltration , astrocytic hyperplasia,neovascularization and cerebral edema in the resecting margins of the brain. ②The cells in the resecting margins were dense in comparison with that in control groups（P〈0.05）. Conclusion The changes of ADC value of the resecting margins are correlated with the time of postoperation. The pathologic foundations of which include astrocytic hyperplasia,neovascularization and cerebral edema.
