一次量氯氮平对女性精神分裂症患者脑电活动影响的研究

摘　　要：目的　通过一次量氯氮平对女性精神分裂症患者脑电活动的影响 ,预测该药对女性精神分裂症患者的近期疗效。方法　采用自身前后对照方法 ,比较女性精神分裂症患者服用一次量 (2 5mg)氯氮平前及后 1、2、3、4、5、6h脑电能量变化 ,并动态观察了血药浓度与脑电活动变化的关系 ,数据用方差分析及相关分析。结果　口服一次量 2 5mg氯氮平后 ,脑电图呈中高幅的精神分裂症患者 ,O1导联α2 频段能量在第 5小时较服药前显著降低 (P <0 .0 5 ) ,P3 导联α2 频段能量仅有下降趋势 ,F4 导联δ频段、F3导联 β频段能量仅有增高趋势 ,但未达显著水平。脑电图呈中低幅的精神分裂症患者 ,F4 导联δ频段能量在第 5小时较服药前显著增高 (P <0 .0 5 ) ,O1导联α2 频段能量仅有降低趋势 ,F3 导联 β频段能量有增高趋势 ,但未达到显著水平 ,出现上述脑电变化者 ,临床疗效好 ,反之 ,疗效差。口服 2 5mg氯氮平后血药浓度与反应良好组脑电α2 频段能量变化有相关关系。结论　口服一次量氯氮平后 ,脑电变化明显者对氯氮平的近期临床疗效好。Objective To forecast the shortterm therapeutic effect of clozapine on female schizophrenics by effect of single clozapine on the electrical activity of brain(EAB).Methods The change of brain energy in schizophrenia were compared between preadministration and 1st,2nd,3rd,4th,5th,6th hours after taking single 25mg clozapine using self pre-post control method,and the relationship between pharmaco-concentration in blood and the changes of EAB was dynamically observed.The datum analysed with Ftest and correlation.Results After taking one single 25mg clozapine,in the schizophrenia whose EEG demonstrated moderatehigh amplitude,the α 2 band energy of O 1 lead was significantly lower at 5th hour than preadministration( P <0.05),α 2 band one of P 3 lead had dropping trend,δ band one of F 4 lead and β band one of F 3 lead had increasing trend,but not reach significant level,as for the schizophrenics whose EEG demonstrated moderate-low amplitude,the δ band energy of F 4 lead was significantly higher at 5th hour than preadministration( P <0.05),α 2 band one of O 1 lead had dropping trend,β band one of F 3 lead had increasing trend,but not reach significant level.Patients had above-mentioned EAB change,their curative effect was good;otherwise,bad.Taking orally a single does of clozapine,there was correlativity between the pharmaco-concentration in blood and the change of EEG α 2 band energy in well-responsive group.Conclusion The patients of EEG visibl change with schizophrenia after taking one single clozapine had good short-term clinical efficacy.
