摘 要:目的 研究氯氮平同传统抗精神病药物在治疗中是否会增加患糖尿病和糖耐量减低的危险。方法 对 77例用氯氮平和 82例用传统抗精神病药物治疗的精神分裂症病人进行空腹血糖检查和口服葡萄糖耐量实验 ,按国际卫生组织标准对糖尿病和糖耐量减低进行分类。结果 在用氯氮平治疗的病人中 ,有糖尿病 (Ⅱ型 )者为 6 .49% (5 / 77) ,糖耐量减低者为 19.48% (15 / 77)。用传统抗精神病药物治疗的病人中 ,有糖尿病 (Ⅱ型 )者为 3 .6 5 % (3/ 82 ) ,糖耐量减低者为6 .0 9% (5 / 82 ) ,两组间患糖尿病机率无显著性差异 ,而糖耐量减低 ,两组间则具有显著性差异 (P <0 .0 5 )。结论 氯氮平比传统抗精神病药物更易导致糖耐量减低。Objective To study clozapine and conventional neuroleptic medications if they can increase the risk of developing diabetes and impaired glucose tolerence(IGT)in the course of treating schizophrenic patients.Method The authors examined 77 schizophrenic inpatients treated with clozapine and 82 schizophrenic inpatients treated with conventional neurolepitc medications for fast blood glucose and oral glucose tolerance test.All inpatients are diagnosed according to WHO.Results In inpatients treated with clozapine ,the prevalence rate of type 2 diabetes,IGT were 6.4%(5/77),19.48%(15/77) respectively.In inpatients treated with conventional neuroleptic medications,the prevalence rate of type 2 diabetes,IGT were 3.65%(3/82),6.09(5/82) respectively.There was not significantly different between the two group developing diabetes,significantly different between two groups developing IGT.Conclusion Clozapine is more easily to induce IGT than conventional neuroleptic medications.
