非小细胞肺癌患者血清CYFRA21-1水平检测对预后评估的意义

摘　　要：目的评价初诊非小细胞肺癌(NSCLC)患者血清CYFRA21-1水平检测对预后评估的意义。方法回顾性分析2006年1月至2008年1月期间的NSCLC初诊患者157例,提取患者就诊时的血清CYFRA21-1浓度以及性别、年龄、临床分期、肿瘤大小、组织分化程度等临床数据,随访的终点为NSCLC相关的死亡事件。分析157例初诊NSCLC患者血清CYFRA21-1水平与临床分期、组织分化程度的关系,采用Kaplan-Meier生存曲线分析初诊患者血清CYFRA21-1水平与NSCLC患者发生死亡事件的关系。结果初诊NSCLC患者血清CYFRA21-1水平随着肿瘤临床分期递进而增加,随着肿瘤组织分化程度的增加而逐渐降低。血清CYFRA21-1≤43.5ng/mL的患者,其远期预后优于CYFRA21-1>43.5ng/mL的患者。结论 CYFRA21-1对NSCLC预后评估有一定价值,血清CYFRA21-1水平是影响NSCLC患者预后的危险因素。Objective To evaluate the prognosis value of initial serum CYFRA21-1level in non-small-cell lung cancer patients（NSCLC）.Methods The clinical data of 157patients with NSCLC in Wuxi No.2People＇s Hospital of wuxi from Jan.2006to Jan.2008were retrospectively reviewed.The clinical and laboratory parameters,including the serum CYFRA21-1level,clinical stage,tumor tissue differentiation grade,were recollected from hospital information system and laboratory information system.The association between initial serum CYFRA21-1level with clinical stage,as well as tumor tissue differentiation grade,was retrospectively analyzed in 157non-small-cell lung cancer patients.Kaplan-Meier survive curve was used to assess the prognosis value of initial serum CYFRA21-1level in non-small-cell lung cancer patients.Results The serum CYFRA21-1level in non-small-cell lung cancer patients was increased with the progress of grade,but decreased with the progress of tumor cell differentiation grade.Lower serum CYFRA21-1level（≤43.5ng/mL）was associated with higher survive rate than that of higher serum CYFRA21-1leve1.Conclusion The initial serum CYFRA21-1level was an risk prognosis factor for patients with NSCLC.
