C-erbB-2、Ki67、nm23基因表达与乳腺癌腋窝淋巴结转移的相关性分析

摘　　要：目的探讨乳腺癌C-erbB-2、Ki67、nm23基因表达与腋窝淋巴结转移的相关性及判断乳腺癌预后价值的临床意义。方法对260例乳腺癌术后组织标本采用免疫组化S-P法进行C-erbB-2、Ki67、nm23基因检测。结果在260例中C-erbB-2、Ki67、nm23的阳性表达率分别为54.62%、53.85%、53.08%。在有腋窝淋巴结转移的152例(152/260)中,C-erbB-2的阳性表达率为77.63%;在无腋窝淋巴结转移组108例(108/260)中C-erbB-2的阳性表达率为20.37%(OR=13.57,P<0.01);Ki67在两组中的阳性表达率分别为71.05%、29.63%(OR=5.58,P<0.01);nm23在两组中阳性表达率分别为40.79%、70.37%(OR=0.29,P<0.01)。结论 C-erbB-2、Ki67在乳腺癌中的阳性表达与乳腺癌的腋窝淋巴结转移呈正相关,nm23基因阳性表达与乳腺癌腋窝淋巴结转移呈负相关,检测C-erbB-2、Ki67及nm23基因的表达情况并结合腋窝淋巴结是否转移等,综合分析它们之间的相互关系,不仅是评估乳腺癌患者预后的重要指标,而且还有助于选择制定合理的治疗方案。Objective To explore the relation between the gene expression of C-erbB-2,Ki67,nm23 and axillary lymph node metastasis in breast cancer,and to study their clinical significance in evaluating the prognoses of patients with breast cancer.Methods Immunohistochemical staining method was used to determine the gene expression of C-erbB-2,Ki67,and nm23 in the postoperative specimens from 260 cases of breast cancer.Results Among the 260 cases,the positive expression rates of C-erbB-2,Ki67,and nm23 were 54.62%,53.85%,and 53.08%,respectively.The positive expression rate of C-erbB-2 in 152 cases of axillary lymph node metastasis was 77.63%,while the positive expression rate of C-erbB-2 in 108 cases without axillary lymph node metastasis was 20.37%（OR=13.57,P0.01）.The positive expression rates of Ki67 of the two groups were 71.05% and 29.63%,respectively（OR=5.58,P0.01）.The positive expression rates of nm23 of the two groups were 40.79% and 70.37%,respectively（OR=0.29,P0.01）.Conclusions The positive expression of C-erbB-2 and Ki67 in breast cancer is positively correlated with axillary lymph node metastasis,while the positive expression of nm23 is negatively related to axillary lymph node metastasis in breast cancer.Determining the gene expression of C-erbB-2,Ki67 and nm23,judging the metastasis of axillary lymph nodes,and comprehensively analyzing the relationship among them in breast cancer are not only the important indicators for assessing the prognoses of the patients with breast cancer,but they are also conducive to selecting and formulating the rational therapy.
