玻璃体切割术治疗球内异物28眼分析

摘　　要：目的探讨玻璃体切割术治疗球内异物的效果。方法对解放军白求恩国际和平医院眼科2007年4月—2011年9月行玻璃体切割术治疗的球内异物27例(28眼)的临床资料进行回顾性分析。结果本组成功取出异物27例(28眼),异物取出率100%。达到功能治愈18眼(64.3%),达到解剖治愈9眼(32.1%),未愈1眼(3.6%);25眼(89.3%)视力提高,3眼(10.7%)视力无提高,其中2眼为术前即无光感。术后随访3个月～4年,有4眼因白内障或晶体缺失行人工晶体植入术,3眼行硅油取出术,1眼发生眼球萎缩。结论玻璃体切割术是目前治疗球内异物、尤其是复杂性眼外伤伴球内异物的最佳方法。选择合适的手术时机并综合应用多种治疗的手段可提高手术成功率。Objective To explore the effect of vitrectomy in treatment of intraocular foreign body.Methods Clinical data of 27 cases（28 eyes） of vitrectomy in treatment of intraocular foreign body during April 2007 and September 2011 in our hospital were retrospectively analyzed.Results All the 28 eyes were performed vitrectomy successfully,and the removal rate was 100.0%.There were 18 eyes（64.3%） of functional cure,9 eyes（32.1%） of anatomic cure,and 1 eye（3.6%） without cure.There were 25 eyes（89.3%） with improved vision and 3 eyes（10.7%） with unimproved vision including 2 eyes of preoperative no light perception.After the surgery,4 eyes underwent intraocular lens implantation due to cataract or absence of lens,3 eyes underwent silicon oil removal surgery and 1 eye had atrophia bulbi for 3 months-4 years of follow-up.Conclusion At present vitrectomy is the best method in treatment of intraocular foreign body especially for complicated ocular trauma with intraocular foreign bodies.To choose the suitable surgery time and comprehensive application of a variety therapy may improve the achievement ratio of surgery.
