抗肿瘤治疗对晚期肿瘤患者外周血CD4+/CD8+记忆T细胞亚群的影响

摘　　要：目的探索建立晚期肿瘤的免疫评估方法,并初步研究抗肿瘤治疗特别是免疫治疗对免疫记忆的影响。方法将31例晚期肿瘤患者按治疗与否分为未治组(17例)与经治组(14例),应用流式细胞术检测患者外周血CD4+中央型记忆T细胞(TCM)、CD8+TCM、CD4+效应型记忆T细胞(TEM)及CD8+TEM比例,分析晚期肿瘤患者免疫记忆特征及免疫治疗对免疫记忆的影响。结果 CD4+T细胞中以TCM为主,而CD8+T细胞中TCM、TEM比例相当。与未治组比较,经治组外周血中CD4+TCM和CD8+TCM比例显著降低(P<0.01,P<0.05),CD8+TEM显著升高(P<0.05)。进一步分析发现免疫治疗次数和患者CD8+TCM比例呈显著正相关(r=0.747,P=0.002)。结论记忆T细胞可有效评价晚期肿瘤患者的免疫状态,且免疫治疗可以有效纠正晚期肿瘤患者的免疫耗竭状态。Objective To establish the immune assessment methods of advanced tumors and preliminarily study the effect of anti-tumor therapy,especially immunotherapy on the immunological memory.Methods A total of 31 patients with advanced cancer were divided into non-therapy group（n = 17） and therapy group（n = 14）.The percentages of CD4 ＋ TCM,CD8 ＋ TCM,CD4 ＋ TEM and CD8 ＋ TEM in the peripheral blood of the 31 patients were detected by flow cytometry,characteristics in advanced cancer patients and influence of treatments on immune memory were analyzed.Results TCM was the most one in CD4 ＋ T cells,while TCM was the same with TEM in CD8 ＋ T cells.Percentages of CD4 ＋ TCM and CD8 ＋ TCM in peripheral blood in therapy group were significantly lower than those of in non-therapy group（P 0.01,P 0.05）,while CD8 ＋ TEM was significantly increased（P 0.05）.Further analysis revealed that immunization frequency and CD8 ＋ TCM proportion was positively correlated（r = 0.747,P = 0.002）.Conclusion Memory T cells can effectively evaluate the immune status of patients with advanced cancer,and immunotherapy can effectively restore immunodepletion status of patients with advanced cancer.
