生存素、细胞周期素D_1和上皮钙黏附素在大肠癌中的表达及临床意义

摘　　要：目的探讨大肠癌(colorectal carcinomas,CC)组织中生存素(survivin,SVV)、细胞周期素D1(CyclinD1)和上皮钙黏附素(E-cadherin,E-cad)表达及临床意义。方法应用免疫组化技术检测120例CC、30例大肠腺瘤(colorectal adenoma,CA)和20例正常大肠黏膜组织(colorectal mucosaltissues,CMT)中SVV、Cyclin D1和E-cad蛋白的表达,并结合临床病理特征进行分析。结果 CC中SVV和Cyclin D1表达阳性率显著高于CA和CMT(P<0.05),而E-cad表达阳性率显著低于CA和CMT(P<0.05)。SVV、Cyclin D1和E-cad表达与CC Duke's分期、浆膜浸润和淋巴结转移显著相关(P<0.05)。CC中SVV表达与Cyclin D1表达呈显著正相关(r=0.25,P<0.05),SVV、Cyclin D1表达与E-cad表达均呈显著负相关(r=-0.24,r=-0.27,P<0.05)。结论 SVV、Cyclin D1和E-cad表达与CC浸润和转移密切相关,联合检测其表达可作为判断CC预后的客观指标。Objective To investigate the expression and clinical significance of surviving（SVV）,Cyclin D1 and E-cadherin（E-cad） in colorectal carcinoma（CC） tissues.Methods The expressions of SVV,Cyclin D1 and Ecad in 120 pieces of colorectal carcinomas（CC）,30 pieces of colorectal adenoma（CA） and 20 pieces of colorectal mucosal tissue（CMT） were detected by immunohistochemical technique,and were analyzed with related clinicopathological characteristics.Results The positive expression rates of SVV and Cyclin D1 in CC were significantly higher than those of in CA and CMT（P 0.05）,while the positive expression rate of E-cad was significantly lower than that of in CA and CMT（P 0.05）.The expressions of SVV,Cyclin D1 and E-cad were correlated with Duke＇s staging,serosa infiltration and lymph node metastasis of colorectal carcinoma（P 0.05）.The expression of SVV was positively correlated with that of Cyclin D1 in CC（r = 0.25,P 0.05）.The expressions of SVV and Cyclin D1 were negatively correlated with those of E-cad in colorectal carcinoma（r =-0.24,r =-0.27,P 0.05）.Conclusion A close relationships exist between the expressions of SVV,Cyclin D1 and E-cad with infiltration and metastasis in CC.These expressions may be used as prognostic impersonal indicators of CC.
