植入氟尿嘧啶缓释剂预防大肠癌术后复发的疗效观察

摘　　要：目的探讨预防进展期大肠癌术后局部复发和全身转移的新途径。方法将55例Dukes B、C期大肠癌患者按时间分为根治切除加植入氟尿嘧啶组(治疗组)和单纯根治术切除组(对照组),随访3年期间,局部复发和全身转移及生存情况。结果治疗组3年局部复发率和全身转移率均低于对照组。3年局部复发数22例,Dukes B期治疗组和对照组分别为3例和5例,33例Dukes C期治疗组和对照组分别为4例和14例,经统计学分析,Dukes B期,P>0.05,Dukes C期P<0.05;3年全身转移数治疗组29例中有2例,对照组26例中有5例,经统计学分析P>0.05。3年生存数治疗组29例中有23例生存,生存率为79.31%;对照组26例中有12例生存,生存率为46.15%,经统计学分析,P<0.05。结论大肠癌根治术中植入缓解氟尿嘧啶可有效降低局部复发率和全身转移率,可提高3年生存率。Objective To investigate the new ways of the prevention of local recurrence and distant metastasis with advanced colorectal cancer patients.Methods 55 cases of Dukes B and C colorectal cancer patients by the time were divided into the radical resection plus fluorouracil implantation group（treatment group）and a simple radical surgery resection（control group）,during the follow-up period of three years,local recurrence and systemic metastasis and survival time were recorded.Results In the treatment group three-year local recurrence rate and systemic metastasis rates were lower than those of control group.Three-year local recurrence was found in a number of 22 cases,in Dukes B treatment group 3 cases and control group 5 cases,33 cases of Dukes C treatment group and control group were 4 cases and 14 cases.Statistical analysis showed Dukes B（P0.05）,Dukes C（P0.05）.Three-year general recurrence was found in 2 cases of the treatment group,5 cases in the control group（P0.05）.Three-year survival was found in 23 cases in the treatment group cases（79.31%）,12 cases in the control group（46.15%）（P0.05）.Conclusion Radical resection for colorectal cancer in remission of fluorouracil implants can effectively reduce the incidence of local recurrence and systemic metastasis rate,and improve the survival rate of three years.
