摘　　要：目的:探讨焦虑障碍患者的心跳知觉水平及可能的相关因素。方法:选取108例符合美国精神障碍诊断与统计手册第4版(DSM-IV)诊断标准的广泛性焦虑障碍(GAD)患者和35例惊恐障碍(PD)患者,以及正常对照45例。采用心跳知觉心理追踪范式评价焦虑障碍患者及正常对照的心跳知觉水平,用汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)、状态-特质焦虑问卷(STAI-Form Y)及艾森克人格问卷成人版(EPQ)评估评定焦虑、抑郁症状和性格特点。结果:PD组心跳知觉水平高于GAD组和对照组[(75.3±18.2)%vs.(65.5±19.5)%,(59.4±20.2)%,P<0.010],GAD组和对照组差异无统计学意义。三组内男性与女性的心跳知觉水平差异无统计学意义;PD组女性患者心跳知觉水平明显高于对照组和GAD组[(76.3±17.5)%vs.(64.4±19.8)%,(53.3±19.4)%,P<0.050],GAD组高于对照组(P<0.050)。焦虑障碍患者心跳知觉水平与年龄(r=0.22)、首发年龄(r=0.23)、以及EPQ中的L量表分(r=0.17)呈正相关(均P<0.050)。结论:惊恐障碍患者尤其是女性患者的心跳知觉水平高于正常对照及广泛性焦虑障碍患者。年龄和首发年龄较大,倾向具有掩饰或防卫性格特点的焦虑障碍患者的心跳知觉水平可能更高。Objective：To explore the heartbeat perception levels and possible related factors of patients with anxiety disorder. Methods： Totally 108 patients with generalized anxiety disorder （GAD）,35 patients with panic disorder （PD） and 45 normal controls were included into this study.The diagnosis was made according to the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition （DSM-IV）.The heartbeat perception levels were tested by the Mental Tracking Paradigm,and Hamilton Anxiety Scale （HAMA）,Hamilton Depression Scale （HAMD）,State-Trait Anxiety Inwentory （STAI-Form Y）,Eysenck Personality Questionnaire （EPQ） were used to evaluate the symptoms of anxiety and depressive and personality. Results： The levels of the heartbeat perception were higher in the PD group than in the normal control group and GAD group [（75.3±18.2）% vs.（65.5±19.5）%,（59.4±20.2）%,P〈0.010],but there was no significant difference between GAD group and normal control group （P〉0.050）.The heartbeat perception levels of females were higher in the PD group than in the control group and GAD group [（76.3±17.5）% vs.（64.4±19.8）%,（53.3±19.4）%,P〈0.050],and higher in the GAD group than in the normal control group （P〈0.050）.The heartbeat perception levels in the PD and GAD group were positively correlated with age,first-episode age and EPQ-Lscore （r=0.17-0.23,Ps〈0.050）. Conclusion： The heartbeat perception levels of patients with panic disorder,especially female patients,are probably higher than that of patients with generalized anxiety disorder and normal controls.The patients with anxiety disorder with older age or later first-episode age or concealing or retaining character,maybe have higher level of the heartbeat perception.
