从《伤寒论》解读“君火以明,相火以位”及其临床意义  

Exploring the meaning and clinical significance of ‘sovereign-fire lightening and ministerial’ fire functioning from Treaties on Cold Pathogenic Diseases

摘　　要：从《伤寒论》方证解读《黄帝内经》"君火以明,相火以位"的内涵与临床意义。认为"君火以明,相火以位"是中医"心肾相交"理论的基础,并重点从君、相二火的功能及状态失调论述了《伤寒论》中心肾不交的方证及其相关经方的临床运用。文章分4部分阐述:①君火痞塞,独焰于上:大黄黄连泻心汤证;②君火痞塞,相火不应:附子泻心汤证;③君火不明,相火不位:桂枝加桂汤证、茯苓桂枝甘草大枣汤证;④相火不位,肝风内动:真武汤证。The author investigated the theory ‘Ministerial fire operating well is based on monarch fire with normal function’ From Yellow Emperor’ Internal Classic in term of the theory of Treaties on Cold Pathogenic Diseases,and try to explore and summarize the clinical practicing of ‘disharmony between heart and kidney’ theory by analyzing the physiology and pathological mechanism of ministerial fire and monarch fire,which helped to drive the understanding that the theory of ‘ministerial fire operating well is based on monarch fire with normal function’ would make sense of ‘coordination between the heart and kidney’ and build the foundation for it.There are examples which help to explain the theory ‘ministerial fire operating well is based on monarch fire with normal function’ from four aspects in this article.Firstly,Dahuang Huanglian Xiexin Decoction syndrome reflects the blocking status of Monarch Fire,which would bring more fire on the upper of the body.Secondly,Fuzi Xiexin Decoction syndrome reflects the blocking status of monarch fire without response of ministerial fire simultaneously.Thirdly,Guizhi Jia Gui Decoction and Fuling Guizhi Gancao Dazao Decoction syndrome reflects the status of both monarch fire and ministerial fire with abnormal function.Finally,Zhen Wu Decoction syndrome reflects the status of ministerial fire with abnormal function and liver wind agitation.
