乳腺Paget's病的临床病理特征分析

摘　　要：目的:探讨乳腺Paget's病的临床特点、影像学表现及病理特征。方法:回顾性分析2005年1月至2011年1月南京军区福州总医院普外科收治的16例乳腺Paget's病患者的临床资料。结果:16例患者术前均行乳腺钼靶摄片,14例发现乳头乳晕区皮肤增厚、钙化、肿块或肿大的淋巴结。术后病理示15例合并乳腺实质内肿瘤,ER阳性2例,占12.50%;PR阳性1例,占6.25%;C-erbB-2强阳性14例,占87.50%。结论:乳腺Paget's病常合并乳腺实质内肿瘤,乳腺钼靶摄片可以较为准确的发现乳房内病变。多数乳腺Paget's病癌组织ER、PR阴性,C-erbB-2强阳性,这对于该病的辅助治疗具有重要的指导意义。Objective： To determine the clinical, imaging and pathological findings of Paget ＇s disease of the breast. Methods： Retrospective review of the pathological diagnosis of 16 women with Paget＇s disease of the breast at Fuzhou General Hospital of Nanjing Military Command between January 2005 and January 2011. Results：Almost all patients （15/16） had underlying breast malignancies. Mammography was performed in all 16 patients and 14 cases were found gross nipple and breast nodule skin thickening, calcification, lumps or enlarged lymph nodes. The expression rate of ER and PR were 12.50% and 6.25%. The expression rates of C - erbB -2 was 87.50%. Conclusion：Patients with Paget~ disease of the breast have a high incidence of an underlying breast carcinoma and mammography should be performed to identify the underlying breast carcinoma. The majority of breast cancer tissue of Paget＇s disease of ER, PR negative,while the C - erbB - 2 was strongly positive. This has an important guiding significance for the adjuvant treatment of the disease.
