复方谷氨酰胺胶囊防治急性放射性直肠炎临床研究

摘　　要：目的:观察复方谷氨酰胺胶囊对盆腔肿瘤放疗后急性放射性直肠炎的预防作用。方法:将78例盆腔肿瘤放疗患者随机分为两组:对照组36例,放疗时不预防使用保护肠道黏膜药物;治疗组42例,放疗时使用复方谷氨酰胺胶囊口服药物保护肠道黏膜治疗。比较两组出现急性放射性直肠炎时的放疗剂量及放射性肠炎严重程度的差异。结果:对照组患者在盆腔接受>10-20Gy照射时大多数患者(33.33%)出现急性放射性直肠炎,而治疗组57.14%患者出现急性放射性直肠炎时已接受>20-30Gy放疗,治疗组推迟了放射性直肠炎的发生时间。同时急性放射性直肠炎分级分析,对照组0级0人、1级7人(19.44%)、2级13人(36.11%)、3级15人(41.67%)、4级1人(2.78%);治疗组0级0人、1级16人(38.10%)、2级17人(40.48%)、3级9人(21.43%)、4级0人,治疗组出现3级直肠炎的患者明显少于对照组(P<0.05),提示治疗组减轻了放射性直肠炎的发生程度。结论:口服复方谷氨酰胺胶囊可减轻盆腔放疗引起的急性放射性直肠炎,同时可推迟直肠炎发生的时间,安全、有效,值得临床推广使用。Objective： To observe the prevention and therapeutic effect of compound glutamine entero - soluble capsule on acute radiation proctitis. Methods： All 78 cases of pelvic tumor patients treated with radiotherapy were randomly assigned to the control group （36 patients） and the treatment group （42 patients）. In control group 36 cases treated with radiation were not treated with preventive drugs,but the patients in the treatment group were treated with oral compound glutamine entero - soluble capsule ,4 tablets each time, three times a day. The efficacy of treatment and severity of acute radiation proctitis after treatment were evaluated. Results： Most patients （33.33%） of the control group suffered from acute radiation proctitis when they received 〉 10 -20Gy radiotherapy. However,57.14% of the treatment group has accepted 〉 20 - 30Gy radiotherapy when suffering from acute radiation proetitis. Compound gluta- mine entero - soluble capsule delayed the time of occurrence of radiation proctitis. The grade 3 acute radiation procti- tis effective rate in the treatment group was 21.43% （9/42） ,and that in the control group was 41.67% （15/36） ,the difference between the two groups was significant（ P 〈 0.05 ）. The compound glutamine entero - soluble capsule could reduce the incidence of radiation proctitis extent. Conclusion：Oral compound glutamine capsules can reduce pelvic radiotherapy -induced acute radiation proetitis.
