17例卵巢Brenner瘤诊治分析

摘　　要：目的探讨卵巢Brenner瘤的诊治特点。方法回顾性分析17例卵巢Brenner瘤患者的临床资料,并行随访。结果良性15例,恶性2例。良性患者多因B型超声发现盆腔包块就诊(8例),术后偶然发现亦多见(4例);1例患者术前血E2升高;治疗以双侧附件切除为主,术后随访未见复发病例。2例恶性患者均有腹胀,1例伴腹部肿块,1例伴胸腹腔积液;术前血CA125均升高;1例为Ⅲ期卵巢癌,辅以泰素、卡铂治疗,术后3月情况尚可;1例为Ⅳ期卵巢癌,辅以顺铂、VP16腹腔化疗,紫杉醇、顺铂静脉化疗,术后生存期12月。结论卵巢Brenner瘤的诊断依靠病理检查结果,治疗以手术为主;其中紫杉醇及铂类为其主要辅助化疗药物,晚期患者期待新型的靶向治疗。Objective To review the diagnosis and treatment of ovarian Brenner tumor. Methods The clinical data of 17 patients with pathologically confirmed Brenner tumor were collected and analyzed retrospectively. Results Among 17 cases, 15 were benign and only 2 were malignant. Most patients with benign Brenner tumor had a pelvic mass found by ultrasound diagnosis. 4 were discovered incidentally after surgery and 1 in menopause had a high level of E2 in the serum. Bilateral adnexectomy was performed and no recurrence was found during follow-up. Two patients with malignant Brenner tumor had an abdominal distension and a high serum CA125 level preoperatively. Pelvic mass was palpated in one patient and ascites and hydrothorax were detected in the other. One patient with stage m ovarian tumor received taxol and carboplatin for chemotherapy after primary cytoreductive surgery and was in good condition during the follow-up of three months. Another patient with stage IV tumor received intraperitoneal chemotherapy with cisplatin/VP16 before and after surgery and intravenous chemotherapy with paclitaxel/cisplatin after surgery and died after 12 months. Conclusions Diagnosis of ovarian Brenner tumor depends on pathological examination. Surgical resection is the main therapy and paclitaxel and platinum are the main adjuvant chemotherapy drugs for the disease.
