头颈部低度恶性肌纤维母细胞肉瘤的临床分析

摘　　要：目的探讨低度恶性肌纤维母细胞肉瘤的临床特点、治疗及预后。方法分析1例头颈部低度恶性肌纤维母细胞肉瘤的临床、治疗及随访情况,并复习国内外有关低度恶性肌纤维母细胞肉瘤文献报告。结果患者因右面侧深部肿块接受手术治疗,病理诊断为低度恶性肌纤维母细胞肉瘤,术后半年局部复发再次手术,11月后再次局部复发,再行手术治疗后至今未出现复发转移。国内外文献显示该肿瘤好发于头颈部,根据光学显微镜、免疫组织化学等技术可诊断该病。治疗方法主要为手术切除,术后存在局部复发及远处转移的可能。结论低度恶性肌纤维母细胞肉瘤具有局部侵袭性,可术后复发及远处转移,一旦病理确诊,应扩大切除,并长期随访。Objective To investigate the clinical feature, therapy and prognosis of low grade myofibroblastic sarcoma. Methods A case of low grade myofibrolastic sarcoma in head and neck regions and the related literatures were reviewed. Results The patient received surgical treatment. Recurrences occurred at 6 months and 11 months postoperation. An expand excision on the recurred lesion was performed. There was no symptom of recurrence in the 3-year follow up. Low-grade myofibroblastie sarcoma is a rare tumor with a predilection for head and neck sites,it can recur and metastasize. Conclusion Low grade myofibroblastic sarcoma exhibites aggressive growth, local recurrence, and sometimes distant metastasis. It needs excision with tumor-free margin and long time follow up.
