耳后入路行腮腺区域性切除的临床效果分析

摘　　要：目的:探讨耳后入路行腮腺区域性切除的可行性及美容效果。方法:对31例腮腺下极浅叶良性肿瘤(多形性腺瘤15例,沃辛瘤11例,基底细胞腺瘤2例,脂肪瘤2例,淋巴管畸形1例)患者,采用耳后入路行腮腺区域性切除。结果:全部患者的腮腺病变被完整切除,切口愈合良好,无面神经下颌缘支麻痹,无涎液潴留、涎瘘发生及Frey征出现。经6个月～5年(平均26个月)追踪复查,肿瘤无复发,切口瘢痕隐蔽,美容效果非常好。术后有2例患者出现耳垂麻木,随访3个月后,麻木消失。结论:经耳后入路切除腮腺良性病损是可行的,可获得较理想的美容效果。Objective To evaluate the feasibleness and the comestic outcome of retroauricular approach for regional parotidectomy. Methods 31 cases clinical diagnosis of benign neoplasms located in inferior superficial part of the parotid gland were the indication of regional parotidectomy （pleomorphic adenoma in 15 patients,Warthin＇s tumor in 11 patients, basal cell adenoma in 2 patient, lipoma in 2 patient,and lymphatic malformationin in 1 patient.）AII the cases were performed via retroauricular approach. Results Thirty one regional parotidectomy were successfully performed.All the wounds healed without any problem.No patient suffered from weakness of the marginal mandibular branch of the facial nerve. No cases of salivary fistula and retention were found. The patients were followed up for an average period of twenty-six months.During the follow-up period,no recurrence of the tumors occurred.No one complained of the symptoms of Frye＇s syndrome.The incisions scar was invisible.All the patients showed good aesthetic results.However numbness around the earlobe was observed intwo patients,both of whom recovered within 3 months after surgery.Conclusion Regional parotidectomy by retroauricular approach is feasible and can provide an acceptable cosmetic outcome for benign lesions located in the inferior superficial part of the parotid gland.
