巨块型宫颈癌术前^(252)锎中子后装放疗的临床观察

摘　　要：[目的]研究252锎(252Cf)中子腔内后装放疗在巨块型宫颈癌术前辅助放疗中的作用。[方法]收集2007年1月至2012年6月接受根治性手术治疗的Ⅰb～Ⅱa期宫颈癌病例共102例,术前放疗组33例,宫颈肿瘤直径≥4cm,术前接受252Cf腔内后装放疗;直接手术组69例,宫颈肿瘤直径<4cm,未接受术前辅助治疗。[结果]术前252Cf中子腔内后装放疗的有效率为90.9%,病理完全缓解率24.2%。术前放疗组与直接手术组比较,手术时间为(4.3±1.0)hvs(4.2±0.9)h,术后尿潴留发生率为18.2%vs18.8%,差异无统计学意义(P>0.05);术前放疗组术中出血量增多,(784.9±537.0)mlvs(578.9±311.4)ml,差异具有统计学意义(P<0.05)。[结论]巨块型宫颈癌术前252Cf中子腔内后装放疗的有效率高,部分达到病理缓解,不良反应可以接受,对远期疗效的影响有待随访观察。[Purpose] To assess the role of 252Californium（252Cf） brachytherapy in preoperative radiotherapy on bulky cervical cancer.[Methods] One hundred and two patients with cervical cancer stage Ib～IIa treated by radical hysterectomy between January 2007 and June 2012 were included in this study.Of the 102 patients,33 cases with tumor size ≥4cm received preoperative 252Cf brachytherapy,and 69 cases with tumor size 4cm received surgery without preoperative radiotherapy.[Results] The response rate of preoperative 252Cf brachytherapy was 90.9%,with a pathological complete remission（pCR） rate of 24.2%.The operating times were not significantly different between the preoperative 252Cf brachytherapy group and the surgery alone group（4.3±1.0h vs 4.2±0.9h,P0.05）,also the incidence of urinary retention（18.2% vs 18.8%,P0.05）.The preoperative 252Cf brachytherapy group had significantly more mean intra-operative blood loss compared with the surgery alone group（784.9±537.0ml vs 578.9±311.4ml,P0.05）.[Conclusion] The 252Cf brachytherapy is effective in the preoperation therapy for bulky cervical carcinoma with tolerable toxicity.But the long term response of preoperative 252Cf brachytherapy needs further investigation.
