摘 要:目的 :探讨微血管计数 (MVC)和上皮钙粘附素 (E- CD)表达与膀胱癌恶性程度、浸润及复发的关系。方法 :应用免疫组织化学 SABC法检测 81例膀胱移行细胞癌组织 MVC和 E- CD的表达情况。结果 :1组织学分级 级和 级间的 MVC没有显著性差异 ,但两者与 级比较均有显著性差异 (P均 <0 .0 1)。浸润性癌和复发组膀胱癌的 MVC分别显著高于浅表性癌 (P<0 .0 1)和未复发组膀胱癌 (P<0 .0 1)。 2 E- CD的异常表达与膀胱癌临床分期、病理分级有显著关系 (P均 <0 .0 5 ) ,复发组 E- CD异常表达率 (75 .8% )显著高于未复发组的异常表达率 (31.2 % ,P<0 .0 1)。3E- CD正常表达的膀胱癌的 MVC显著低于 E- CD异常表达的膀胱癌 (P<0 .0 1)。结论 :MVC和 E- CD均可作为膀胱癌恶性程度、浸润及复发的预测指标 ,E- CD可能在肿瘤血管生成方面起一定的抑制作用。Purpose:To determine the expression of E-CD and microvessel count (MVC) in bladder cancer and their relations with malignancy,infiltration and recurrence.Methods: An avidin-biotin immunoperoxidase technique was used to examine the immunostaining of E-CD and MVC in 81 TCC. Results: ① MVC in grade Ⅲ tissue was significantly higher than those in grade Ⅰ and Ⅱ (P< 0.01 ,P< 0.01 ),MVC in tumor tissues of patients with recurrence was significantly higher than in those without recurrence (P< 0.01 ). There was also a significant difference of MVC between superficial and infiltrative lesions (P< 0.01 ).② Decreased E-CD expression correlated with both increased grade and stage (P< 0.05 ,P< 0.05 ).Moreover abnormal E-CD expression correlated well with the high recurrence rate(P< 0.01 ).③ MVC in tussues of patients with abnormal E-CD expression was significantly higher than in those with normal E-CD expression. Conclusions: Detection of E-CD expression and MVC might have practical significance in assessing the malignancy,recurrence and prognosis of bladder cancer. Cellular adhesion molecular E-CD might be an important suppressor in angiogenesis.
