南通市成人体检中原发性肝癌的发病情况变化

摘　　要：目的调查南通市体检者中原发性肝癌的流行趋势。方法从南通大学附属医院体检信息系统中抽取2009年1月～2011年12月成人体检者157554例作为研究对象，157554例中肝超声检查为124313例，发现肝肿瘤．进行CT或MRI，结合乙肝病史和甲胎蛋白异常升高，确定原发性肝癌。结果①157554例体检者中施行肝彩超124313例。检出原发性肝癌新发病例19例，其中2009年9例，原发性肝癌发病率为0．0188％（9／47818），2010年6例，其发病率为0．0134％（6／44927），和2011年4例，其发病率为0．0062％（4／64809），呈逐年下降趋势。②19例中，男16例，女3例，50～〈60岁组以上多见，HBsAg（＋）或HBeAg（＋）或乙肝史9例（47．37％）。非手术治疗15例（78．94％），手术4例（21．06％）。结论南通市体检者中原发性肝癌是一个常见的恶性肿瘤．但逐年来呈下降趋势。Objective To investigate the prevalence of the primary liver cancer for the Nantongs health check-up Subjects. Methods Data of 157 554 Nantong health check up subjects from January 2009 to December 2011 were investigated. Of 157 554 cases, there were data of 19 primary liver cancer of 124 313 liver ultrasonic examination and associated with CT and/or MRI of the liver, AFP abnormal rise and hepatitis B virus history or HBsAg and/or HBeAg positive in health check-up subjects. Results In 157 554 health check-up subject, there were 124 313 ultrasonic examination of the liver. Of 124 313 cases, 19 cases with new primary liver cancer were detected, including 9 cases in 2009, and 6 cases in 2010, and 4 cases in 2011. The prevalence of primary liver cancer, there was 0.0188% （9/47 818） in 2009, and 0.0134% （6/44 929） in 2010, and 0.0062% （4/64 809） in 2011. Of 19 cases, there were 16 men and 3 women, and were 9 cases （47.37%） with AFP abnormal rise, hepatitis B virus history and HBsAg and/or HBeAg positive, and were 4 cases （21.06%） with operative treatment and 15 cases （78.94%） with no-operative treatment. Conclusion The primary liver cancer is a common malignant tumor, but the prevalence of the primary liver cancer is decreasing for health check-up subject.
