低氧放疗对中晚期食管癌的疗效研究

摘　　要：目的探讨低氧放疗对中晚期食管癌的近远期疗效及不良反应。方法将66例欲接受根治性放疗的食管癌患者随机分为低氧组和空气组。所有患者均行三维适形放射治疗,1.8～2.0Gy/次,每周5次,总剂量为64～66Gy。低氧组在放射治疗时吸入10.5%低氧气体,空气组在放射治疗过程中吸入空气。观察两组患者的总有效率、远处转移率、远期生存率和放射治疗反应。结果低氧组和空气组有效率分别为68.9%和71.9%(2=0.0749,P=0.7844),1年生存率分别为75.0%和59.4%(2=1.7719,P=0.1832),3年生存率分别为56.3%和31.3%(2=4.0635,P=0.0438),5年生存率分别为34.4%和12.5%(2=4.2667,P=0.0389)。低氧组的放射治疗急性反应和远处转移率均低于空气组。结论低氧放疗能提高食管癌患者的远期生存率,并能减少放射治疗的不良反应。Objective To study the clinical effects and side effects of hypoxia radiotherapy in patients with middle-late esophageal cancer.Methods 66 patients with esophageal cancer who would receive radical radiotherapy were randomly divided into hypoxia group and air group.Patients in hypoxia group were inhaling 10.5% hypoxia gas during radiation,while the others in air group inhaling fresh air.Both groups were treated with three dimensional conformal radiotherapy,with 1.8-2.0Gy per day and 5 times per week and total dose ranges from 64-66Gy.Observation targets included overall remission rates,distant metastasis rates,long-term survival rates and side-effects.Results Overall remission rates of hypoxia group and air group were 68.9% and 71.9% respectively（2=0.0749,P=0.7844）,1-year survival rates of two groups were 75.0% and 59.4% respectively（2=1.7719,P=0.1832）,3-year survival rates of two groups were 56.3% and 31.3% respectively（2=4.0635,P=0.0438） and 5-year survival rates of those were 34.4% and 12.5% respectively（2=4.2667,P=0.0389）.The radiation acute reactions and distant metastasis rates of hypoxia group were lower than air group.Conclusions Hypoxia radiotherapy can improve the long-term survival rates of patients with middle-late esophageal cancer,and reduce the adverse reactions of radiation.
