结直肠癌患者外周血人类端粒酶反转录酶及基质金属蛋白酶-7mRNA的表达及其相关性研究

摘　　要：目的应用实时荧光定量反转录一聚合酶链反应（RT。PCR）法检测结直肠癌患者外周血中人类端粒酶反转录酶（hTERT）及基质金属蛋白酶-7（MMP-7）mRNA的表达情况，探讨其对监测结直肠癌血液微转移的临床意义及二者的相关性。方法采用TRIzol法提取血液标本中的总RNA，将其反转录成cDNA，然后用RT—PCR技术检测血液标本中MMP-7mRNA和hTERTmRNA表达，计算样本的△cf值。结果在181例结直肠癌患者中，hTERTmRNA表达阳性63例，阴性118例。MMP-7mRNA表达阳性61例，阴性120例。不同性别、不同年龄的结直肠癌患者外周血hTERT、MMP．7mRNA表达阳性率差异无统计学意义（P〉0．05）。低分化腺癌患者MMP-7mRNA表达阳性率（43．9％）较中高分化腺癌（27盘％）增高（x^2=4．873，P〈0．05）。Ⅲ、Ⅳ期患者外周血hTERTmRNA表达阳性率（42．5％）较I、Ⅱ期患者（24．0％）增高（X^2=6．591，P〈0．05）。Ⅲ、Ⅳ期患者外周血MMP．7mRNA表达阳性率（39．6％）较I、Ⅱ期患者（25-3％）增高（X。4．014，P〈0．05）。有远处转移的结直肠癌患者外周血hTERTmRNA表达阳性率（53．3％）较无远处转移的患者（28．7％）增高（x^2=9．059，P〈0．05）。有远处转移的结直肠癌患者外周血MMP-7mRNA表达阳性率（46．7％）较无远处转移的患者（29．4％）增高（x^2=4．506，P〈0．05）。CEA阳性患者外周血hTERTmRNA表达阳性率（52．6％）较CEA阴性的患者（30．1％）明显增高（x。6．735，P〈0．05）。CEA阳性患者外周血MMP-7mRNA表达阳性率（60．5％）较CEA阴性的患者（26．6％）明显增高（x^2=15．490，P〈0．05）。无远处转移的结直肠癌患者中，以hTERT、MMP-7mRNA一项高于正常值范同为血液微转移表达阳性。检测后6个月微转移表达阳性组肿瘤复发转移率（36．58％）与阴性组（7．14％）比较增高（x。13．027，P〈0．05）；检测后12Objective To detect the expressions of hTERT and MMP-7 mRNA in the peripheral blood of patients with colorectal cancer and their correlation, and discuss the clinical significance of peripheral blood micro-metastasis. Methods Total RNA was extracted from blood samples by TRIzol and was reverse transcribed into cDNA. The mRNA expressions of MMP-7 and hTERT were detected by real-time PCR. Results Positive mRNA expression of hTERT was found in 63 of 181 patients with colorectal cancer, while positive mRNA expression of MMP-7 was seen in 61 cases. The positive rates of hTERT and MMP-7 mRNA had no significant differences in the patients＂ sex and age （P 〉 0.05）. The positive rate of MMP-7 mRNA in the patients with poorly differentiated adenocarcinoma （43.9 %） was higher than that in the patients with moderately and well differentiated adenocarcinoma （27.8 %） （X2 = 4.873, P 〈 0.05）. The positive rate of HTERT mRNA in the patients with stage Ⅲ-Ⅳ （42.5 %） was higher than that in the patients with stage Ⅰ - Ⅱ （24.0 %） 0（2 = 6.591, P 〈 0.05）. The positive rate of MMP-7 mRNA in the patients with stage Ⅲ-Ⅳ （39.6 %） was higher than that in the patients with stage Ⅰ - Ⅱ （25.3 %） 0（2 = 4.014, P 〈 0.05）. The positive rate of hTERT mRNA in the patients with distant metastasis （53.3 %） was higher than that in the patients without distant metastasis （28.7 %） 0（2 = 9.059, P 〈 0.05）. The positive rate of MMP-7 mRNA in the patients with distant metastasis（46.7 ＇%） was higher than that in the patients without distant metastasis （29.4 %） （X 2 = 4.506, P 〈 0.05）. The positive rate of hTERT mRNA in the patients with positive CEA expression （52.6 %） was significantly higher than that in the patients with negative CEA expression （30.1%） 0（2 = 6.735, P 〈 0.05）. The positive rate of MMP-7mRNA in the patients with positive CEA expression （60.5 %） was significantly higher than those in the patients with negative CEA expression （26.
