乳腺癌保乳治疗美学效果的相关因素分析

摘　　要：目的探讨乳腺癌保乳治疗对美学效果的影响因素。方法接受保乳治疗的女性乳腺癌患者230例,放疗后6个月以乳头横向移位距离、乳头纵向移位距离、乳房凹陷程度、乳房质地与弹性及皮肤色泽等为指标,评价美学效果。结果 230例患者均行单侧手术,美学效果评价良好率为69.56%(160/230),一般26.96%(62/230),差3.48%(8/230)。采用双切口、保留皮肤和肿瘤局部扩大切除的手术技巧以及适形放疗,可提高保乳术后美学效果(P<0.05),而肿瘤分期越早,其术后美学效果越好(P<0.01)。术后放疗开始时间对保乳治疗后美学效果良好率影响不明显(P>0.05)。结论早期乳腺癌保乳治疗中,采用双切口、保留皮肤和局部扩大切除以及适形放疗,有助于改善美学效果。Objective To explore the impact factors associated with aesthetic effect of breast-conserving therapy for breast cancer patients. Methods A total of 230 breast cancer patients received breast-conserving treatment. Six months after radiotherapy, taking the distances of nipple lateral and vertical displacement, degree of surface depression, breast and skin texture, elasticity and color as indexes, a comprehensive evaluation on the aesthetic effect of breast conserving treatment was done. Results All 230 cases of breast-conserving surgery were unilateral. The comprehensive evaluation of aesthetic effect showed that 160 cases were good which accounted for 69. 56% （160/230） ,62 cases general accounted for 26.96% （62/230） and 8 cases poor accounted for 3.48% （8/230）. Surgical techniques such as dual incision, reserve skin, extensive tumor resection, and radiotherapy could improve the aesthetic effect of breast-conserving surgery （P 〈0.05）. Earlier tumor stage,better aesthetic effect after surgery（P 〈0.01 ）. The start time of postoperative radiotherapy had little impact on aesthetic effect after breast-conserving surgery （P 〉 0.05）. Conclusion Application of the techniques, including dual-incision,reserve skin and extensive tumor resection,and rodiotherapy to early-stage breast cancer conserving theraov can help to improve the aesthetic effect.
