胃癌患者手术前后血清LXA4、TNF-α的检测及意义

摘　　要：【目的】检测胃癌患者手术前后血清中脂氧素A4（LXA4）、肿瘤坏死因子（TNF-a）的水平，探讨二者在胃癌诊断、进展和预后方面的价值。【方法】酶联免疫吸附试验测定82例胃癌患者及同期30例正常对照者血清中LxA4及TNF—α水平，并对胃癌术前术后以及不同分期的患者血清LxA4、TNF-α水平进行比较。【结果】胃癌患者术前血清LXA4、TNF—α水平高于术后及正常对照组，差异具有统计学意义（P〈0．05）；术后血清LXA4、TNF-a水平与正常对照组相比差异无统计学意义（P〉0．05）。Ⅳ期胃癌患者血清LXA4、TNF-a水平高于Ⅰ～Ⅲ期患者（P〈0．05）；Ⅱ、Ⅲ、Ⅳ期之间血清LxA4、TNF-α水平相比较，差异无统计学意义（P〉0．05）。胃癌患者血清I。xA4与TNF-α的水平呈正相关（r=0．251，P=0．004）。【结论1胃癌患者术前血清的LxA4、TNF-α水平增高，并与胃癌的临床分期呈正相关，LXA4及TNF-α的检测对胃癌的诊断及预后有一定的评估作用。[Objective] To detect the level of lipoxinA4（LXA4） and tumor necrosis factor-alpha（TNF-α） in serum of patients with gastric cancer before and after operation, and to explore the value of both of them in the diagnosis, develop- ment and prognosis of gastric cancer. [Methods] Serum LXA4 and TNF-α in 82＇patients with gastric cancer and 30 nor- mal controls were detected by enzyme linked immunosorbent assay（ELISA）. Serum LXA4 and TNF-α in patients with gastric cancer with different stages and before and after operation were compared. [Results] Serum levels of LXA4 and TNF-α in gastric cancer before operation were higher than those after operation and in normal control group, and there was significant difference（ P 〈0.05）. There was no significant difference in serum levels of LXA4 and TNF-α between after operation and the normal control group（ P 〉0.05）. Serum levels of LXA4 and TNF-α in patients with stage IV were higher than those in patients with stagesⅠ-Ⅱ （ P 〉0.05）. There was no significant difference in serum levels of LXA4 and TNF-α among patients with stage Ⅱ, Ⅲ or Ⅳ （P〉0.05）. There was positive correlation between serum LXA4 and TNF-α in patients with gastric cancer（r=0. 251, P =0. 004）. [Conclusion] Serum levels of LXA4 and TNF-a in pa- tients with gastric cancer before operation increase significantly, and have the positive correlation with clinical stage of gas- tric cancer. The detection of LXA4 and TNF-α is of significance in the evaluation of the diagnosis and prognosis in gastric cancer.
