胎儿马蹄肾的产前超声诊断

摘　　要：目的探索胎儿马蹄肾的产前超声诊断方法和技巧,提高产前诊断率。方法对2008年7月至2011年10月期间我科室诊断为胎儿马蹄肾和漏诊的病例进行回顾性分析总结;分析胎儿马蹄肾病例肾脏各切面超声表现特征,并总结出超声诊断线索及要点;对所有病例加以追踪。结果诊断22例胎儿马蹄肾中4例为单一马蹄肾畸形,出生后均证实;其余18例均伴发其他畸形,除1例家属拒绝尸检外,其余均尸检证实;2例漏诊,因其他畸形进行尸检时意外发现。胎儿马蹄肾时双肾横切面显示双肾肾盂角较正常胎儿明显缩小,双肾冠状面扫查发现双肾下极融合形成峡部,腹部横切面显示峡部横跨于腹主动脉前方。结论胎儿马蹄肾并不罕见,产前超声检查时胎儿双肾肾盂角的观察及常规双肾冠状面及肾脏上下极间的腹部横切面腹主动脉前方的观察可提高产前诊断率。Objective To investigate the method and technique in diagnosis of fetal horseshoe kidney. Methods The data of fetuses succeed or failed to diagnosed horseshoe kidney were reviewed and the ultrasound characters of each section were analyzed in order to make a conclusion of diagnostic clue and keys. All cases were traced. Results 22 fetuses were diagnosed horseshoe kidney. 18 of 22 cases were found with other malformations and all of which were confirmed by autopsy except one refused case. 4 eases had no other abnormality. 2 cases were failed to diagnose which were found horseshoe kidney by autopsy because of other malformation. The renal pelvic angles of horseshoe kidney fetuses were much smaller than those of normal, and the lower poles of two kidneys fused in coronal plane and isthmus can be seen across the front of aorta in transversal section of abdomen. Conclusions Horseshoe kidney is not so rare. The observation of renal pelvic angle of kidneys and the lower pole of kidneys in coronal plane and the front of the aorta in transversal section of abdomen can increase the prenatal diagnostic rate.
