摘 要:目的:评价新型光动力抗菌化学疗法(PDT)的根管消毒效果。方法:取新鲜拔除的牛下颌切牙,垂直于牙体长轴将根管切割为冠方、根中及根尖三部分,高压灭菌后固定于培养皿中。冠方、根中1/3样品中加入粪肠球菌(TACC29212)混悬液建立根管感染模型,根尖1/3加入等量的无菌培养基,作为无菌对照。根中1/3标本只进行细菌接种而不进行根管消毒,作为阳性对照;冠方1/3的标本随机分为5组(n=10),分别经30 g/L次氯酸钠液消毒30 s、PACT 200系统消毒4 min及6 min、单独激光照射6 min、单独光敏材料作用6 min后,根管样品纵向剖开,自根管内壁向外0.5 mm处切取新鲜牙本质碎屑,常规琼脂铺板计数,以根中1/3集落形成单位(CFU)为阳性对照,计算抗菌率。结果:新型光动力疗法PACT 200系统消毒6 min可达到73.47%±5.36%的抗菌率,与次氯酸钠组(81.38%±5.26%)(P>0.05);而PACT 4 min组仅达到50.23%±4.34%的抗菌率,显著低于PACT 6 min组和次氯酸钠组(P<0.05);单独激光和单独光敏组均无明显抗菌作用。结论:PACT 200系统消毒6 min可有效杀灭感染根管模型内的细菌,有望成为感染根管消毒的新方法。AIM: To evaluate the disinfection effect of photodynamic therapy (PDT) on a modified artifi- cially infected root canal model. METHOD : The roots of freshly extracted permanent bovine mandibular incisors were sectioned, perpendicularty to the long axis, into coronal, middle and apical parts. All specimens were autoclaved and fixed to Petri dishes. The canals of coronal and middle regions were filled with a suspension of Enterococcus faecalis, while the apical region with culture medium as negative controls. Seven days later, coronal sections were divided into 5 groups ( n = 10 ) and disinfected by 30 g/L NaOC1 for 30 s, a system of photodynamic therapy ( PACT 200 ) for 4 min or 6 min, laser irradiation for 6 min and light irradiation respectively. Dentine chips were obtained at 0.5 mm depths from the central canal to the periphery, and their colony-forming units (CFU) of Enterococcusfaecalis and antibacterial rates were examined. RESULTS: The antibacterial rate of the PACT 200 treatment for 6 rain (73.47% ±5.36% ) was similar to that of the NaOC1 (81.38% +5.26% ) (P 〉0.05), while that of the PACT 200 treatment for 4 min was (50.23 % ± 4.34% ) lower than those in above 2 groups (P 〈 0.05 ). Laser or light irradiation showed no antibacterial effect. CONCLUSION: Under the present experimental conditions, PACT 200 treatment for 6 min has disinfectioneffect for in|~cted root canal.
