摘  要:目的观察早期高压氧联合激素干预对预防一氧化碳中毒大鼠迟发性脯病(DEACMP)发生的影响,探讨DEACMP的发病机制。方法46只雄性Wistm大鼠按数字表法随机分成4组:健康对照组(NC组)10只,一氧化碳中毒组(COP组)12县,一氧化碳_fl毒+高眶氰组(HBO组)12只,一氧化碳中毒+HBO+激素组(DEX组)12只。制备急性一氧化碳中毒动物馍型,采用Morris水迷宫评估比较大鼠智力状态,以连续2次逃逸潜伏期延长认定为发生DEACMP,以逃逸潜伏期延长后的平均时间与该大鼠治疗期间共19d的平均逃逸潜伏时间之差(△逃逸潜伏朗)判定DEACMP的严重程度?结果染毒后各组均出现不同程度逃逸潜伏期延长,总体趋势为染毒岳第3天出现延长,第11天达高峰,随后逐渐下降;△逃逸潜伏期:DEX组为(1.24±1.54)s,HBO组为(2.26±1.16)S,COP组为(6.93±2.86)S,3组比较差异均有统计学意义(P〈0.05);结论早期HBO联合激素治疗较单纯HBO治疗能更明显地减轻DEACMP病变的严重程度,支持DEACMP发病的自身免疫学说。Objective To observe the effect of early HBO therapy combined with hormone intervention on the prevention of delayed eneephalopathy in rats with acute carbon monoxide poisoning (DEACMP) and also to explore the mechanism of DEACMP. Methods Forly-six male Wistar rats were randomly divided into 4 groups: the normal control group (NC) (n = 10) ; the CO poisoning group (COP) (n = 12) ; the CO poisoning + HBO therapy group (n = 12) ; the CO poisoning + HBO + dexamethasone group (DEX) ( n = 12). Then, aeute monoxide poisoning (ACMP) animal model was developed. Morris water maze was used to evaluate and analyze the intelligence levels of rats. Repeated delayed escape lalency for twice in succession was held to have DEACMP. The difference ( △ escape latency) between the average time for delayed escape latency and the average escape latency during treatment for 19 days was used to assess the seriousness of DEACMP. Results Escape latencies of all the intoxication groups delayed to various extents, generally, the escape latency on the 3rd day began to delay, and on the llth day reached peak, then gradually dropped to lower levels. The following were △ eseape lateneies of different groups:△ escape latency for the DEX group was( 1.24 ± 1.54) s, that for the HBO group was (2.26± 1.16) s and that for the COP group was (6.93 ±2.86) s. Significant statistical differences eould be seen between the 3 groups ( P 〈 0.05 ). Conclusions When compared with simple HBO therapy, early ttBO therapy combined with hormone intervention could deerease the seriousness of DEACMP in rats, whieh supported the them7 of autoimnmnity.
