孤立性浆细胞瘤临床及影像学表现分析

摘　　要：目的:分析孤立性浆细胞瘤(SP)的临床及影像学特点,以提高对SP的影像诊断能力。方法:回顾性分析经手术或活检病理证实的7例SP患者的临床影像学资料。结果:7例均为单发病灶。发生于骨4例,临床表现为病变部位肿胀疼痛;影像学表现为膨胀性溶骨性骨质破坏,边界较清晰,骨皮质变薄或断裂,部分伴周围软组织肿块。骨外3例,临床表现为局部不适、异物感、鼻出血等;影像学均表现为软组织肿块,边界清晰,CT平扫肿块密度与肌肉相似。结论:SP患者全身症状不明显,但病灶边界均较清晰。发生于骨的患者临床表现为病变部位肿胀酸痛,影像学表现主要为膨胀性溶骨性骨质破坏伴破坏区和(或)周围软组织肿块;发生于骨外者临床表现因病变部位不同而异,影像学表现为孤立性软组织肿块。Objective： To analyze the clinical and imaging presentation of 7 cases of solitary plasmacytoma and improve the ability of diagnosis and differential diagnosis of the tumors. Methods： Retrospective analysis the clinical and imaging appearances of 7 patients with solitary plasmacytoma confirmed by biopsy or operation.Results： Regarding SP,4 of the tumors were located in the bones, appearing partially painful and swelling and expansively osteolytic with clear borders, thin and broken cortical and soft-tissue masses on CT scan.Other 3 were located outside the bones and all showed soft tissue mass, having the similar density with muscle on unenhanced CT scans.Conclusion： SP lacks symptoms in the whole body,as compared with multiple myeloma,and borders are well defined.Patients of SPB clinically present local pain and swelling, and imaging features are expansive osteolytic destruction of bone and/or soft tissue masses.Patients of EMP,however,often present local soft tissue mass with nonspecific clinical appearance.
