黏弹剂在新生血管性青光眼手术中的应用

摘　　要：目的:探讨黏弹剂在新生血管性青光眼(NVG)行复合式小梁切除术中的应用。方法:对30例(30眼)术中有出血倾向的NVG患者,前房注入黏弹剂止血后完成复合式小梁切除术,术中根据眼压及出血情况酌情从穿刺口放出黏弹剂,术后观察眼压、虹膜面新生血管、视功能、前房及滤过泡,随访半年。结果:28眼眼压控制在21 mmHg以下,有效率达93.3%;术中、术后均无发生前房大量出血;26眼虹膜新生血管消退,26眼视力无变化或有所提高,28眼为功能性滤过泡。结论:NVG行复合式小梁切除术中,应用黏弹剂能有效防止前房大量出血,避免出血阻塞滤过口,提高手术成功率。Objective： To evaluate the effectiveness of Healon agent in complex trabeculectomy for neovascular glaucoma.Methods： Combined trabeculectomy was performed in 30 cases（30 eyes） of neovascular glaucoma with hemorrhagic tendency after Healon agent was infused into anterior chamber.The Healon agent was released according intraocular pressure and the amount of hyphema.The changes of intraocular pressure（IOP）,iris surface neovascularization,visual acuity,anterior chamber and filtering bleb were carefully observed after surgery in the follow-up of 6 months.Results： IOP were controlled under 21 mmHg in 28 cases（28 eyes）,the effective rate was 93.3%.There was no anterior chamber hyphema during the surgery and after the surgery in any cases.Iris surface neovascularization disappeared in 26 cases（26 eyes）,visual acuity increased or unchanged in 26 cases（26 eyes）,the functional blebs were observed in 28 eyes. Conclusion： Using Healon agent during compound trabeculectomy for neovascular glaucoma can effectively reduce the risk of anterior chamber hyphema and avoid obstruction of filtration entrance,thus improves the successful rate of the operation.
