共同性内斜视患儿术前压贴三棱镜对术后双眼视功能的影响

摘　　要：目的探讨压贴三棱镜治疗对共同性内斜视患儿双眼视功能的保护作用，比较术前压贴三棱镜与未行三棱镜治疗的内斜视患儿的术后远双眼视功能及近立体视情况。方法选择自2010年6。12月我院就诊的部分调节性内斜视患儿40例，年龄4-11岁之间，平均5±0．7岁，将病例随机分为两组，实验组20例，对照组20例，实验组配镜后根据戴镜后的三棱镜检查结果给予压贴三棱镜，对照组仅戴镜，所有患者均于戴镜6～12个月后接受斜视矫正手术，手术前后均用同视机检查远双眼视功能、Titmus立体图检查近立体视。所有数据采用SPSS13．0软件进行统计学分析，采用χ2检验Fisher确切概率法，P〈0．05为具有统计学意义。结果实验组与对照组远同时视及融合功能比较，无显著性差异（χ2=0．633，P〉0．05）。同视机远立体视、Titmus近立体视觉比较，实验组术后远立体视功能优于对照组，有显著性差异（χ2分别为8．286、8．120，P〈0．05）。结论内斜视患者术前行压贴三棱镜治疗能保护双眼视功能，同时视及融合功能改善较未戴三棱镜者无明显差异，远近立体视功能比未行三棱镜治疗者好。Objective To observe the effects of Fresnel press-on prism in protecting binocular vision of patiens with concomitant esotropia and to compare postoperative distant binocular vision and near stereoacuity between the children received Fresnel press-on prism treatment and those not received before operation Methods 40 children with concomitant esotropia derived from outpatient clinic since June 2010 to December, age between 4 to 11 years old, mean 5 ±0.7 years, were randomly individed to experimental group（20 cases）and control group（20 cases）. The former were treated with Fresnel press-on prism according to the result of prism after wearing glasses and amblyopia was treated simultaneously. While the latter group were only treated with glasses. All patients accepted strabismus correction operation in wearing glasses 6-12 months later.All the children were examined with synoptophore for disrant binocular vision acuity and with Titmus for near stereoacuity before and aftar surgery. The data were analyzed with SPSS13.0 statistical software for Fisher＇s exact test. Results Distant simultaneous perception and fusion function was no significant difference between experimental group and control group（χ2=0.633,P〉0.05）. Distant synoptophorstereoacuity and close Timtusstereoacuity of experimental group was superior to that of control group and there were significant difference （χ2 =8.286 及 χ2 =8.120, P 〈 0.05）. Conclusions Preoperative Fresnel press-on prism can protect binocular vision of patiens with concomitant esotropia. Simultaneous perception and fusion function was no significant difference in two group, but the stereoacuity of children with Fresnel press-on prism treatment was better than that of without Fresnel press-on prism treatment.
