人类白细胞抗原B27相关性急性前葡萄膜炎临床特征的Logistic回归分析

摘　　要：目的应用Logistic回归寻找人类白细胞抗原B27(human leucocyte antigen-B27,HLA-B27)相关性急性前葡萄膜炎(acute anterior uveitis,AAU)的临床特征。方法应用流式细胞仪对64例AAU患者血液中HLA-B27抗原进行测定,并对HLA-B27抗原阳性和阴性AAU患者的临床特征进行Logistic回归分析。结果受检者中阳性组30例,阴性组34例。其中性别、单眼急性发作、羊脂状角膜后沉着物(keraticprecipitate,KP)、红细胞沉降率、虹膜后粘连、双眼同时急性发作与HLA-B27抗原无统计学上的联系;而双眼交替性急性发作、细小KP、前房渗出、脊柱关节病与HLA-B27抗原存在统计学上的联系。结论 HLA-B27相关性AAU多表现为双眼交替性急性发作、细小KP、前房渗出,伴随脊柱关节病,很少表现为双眼同时急性发作和羊脂状KP。Objective To analyze the clinical features of human leucocyte antigen-B27 （HLA- B27）-associated acute anterior uveitis （AAU） by Logistic regression. Methods Sixty-four patients with AAU were divided into two groups （ the positive group and the negative group） by the HLA-B27 antigen, which was tested by flow cytometer, and the clinical characteristics of two groups were analyzed by Logistic regression. Results There were 30 cases in the positive group, and 34 cases in negative group. By analysis, gender, unilateral eye, both eyes, ＂mutton-fat＂ keratic precipitate, blood sedimentation and posterior synechiae had no correlation with HLA-B27 antigen, and these sign of alternative-eye, fine keratic precipitate, anterior chamber fibrin exudates and systemic arthritic disease had positive correlation with HLA-B27 antigen. Conclusion HLA-B27-associated AAU always shows alternative-eye, fine keratic precipitate, anterior chamber fibrin exudates and systemic arthritic disease, while rarely shows both eyes, ＂mutton-fat＂ keratie precipitate.
