机用根管锉在老年患者创伤性根尖周炎的临床应用

摘　　要：目的观察ProTaperfile机用系统在老年创伤性根尖周炎患者中根管治疗的效果。方法把需要进行治疗的创伤性根尖周炎患者125例,220颗患牙,随机分为机用大锥度proTaper镍钛锉治疗组(P组,前牙64颗,后牙42颗)和手用不锈钢K锉治疗组(H组,前牙73颗,后牙41颗),P组用ProTaper机用镍钛锉冠根向逐步深入法预备管,H组用手用不锈钢锉逐步后退法预备根管,观察两组在根尖引流效果、调效果、根管治疗的完善程度,治疗操作时间,根尖周破坏的愈合程度,牙齿有无折裂的差异。结果 P组根尖周炎治疗效果优于H组的预备效果(P<0.05);两组的充填时间P组优于H组(P<0.05)。结论 ProTaper机用锉在老年创伤性根尖周炎临床治疗中优于手用不锈钢锉。Objective：To investigate the clinical effects of ProTaper file machine system on the root canal treatments of periapical inflammation from ooclusal trauma of old patients. Methods： 125 cases of periapical inflammation from occlusal trauma （with 220 involved teeth） were chosen and the involved teeth were randomly divided into group P （64 front teeth, 42 latter teeth） and group H （73 front teeth, 41 latter teeth）, the machine with the steep taper file of ProTaper nickel tita- nium for group P and the stainless steel K file for group H. The treatment was step forward from crown to roots in group P while it was step back in group H during the root canal preparation. We observed the effect of periapical drainage, the ad- justed occlusion, and the root canal treatment, the treatment time, the recovery degree of periapical lesion and the differ- ence in whether there was odontoclasis. Results： Curative effect of group P was superior to that of group H, and the two groups showed a significant difference （P 〈 0.05 ） ; The time of root filling in group H was longer than that in group P （P 〈 0.05 ）. Conclusion： ProTaper file machine system is superior to the conventional in treating periapical inflammation from occlusal trauma of old patients.
